血清网膜素－1在中心性肥胖伴原发高血压患者的表达及其意义

摘要

Objective:To explore expression of plasma omentin -1 in patients with central obesity (CO) complicated essential hypertension (EH) and its clinical significance .Methods : Serum level of omentin - 1 was measured by enzyme linked immunosorbent assay in 57 CO patients without EH (pure CO group) and 67 CO complicated EH patients (CO + EH group) ,and 56 healthy subjects (normal control group) .Blood glucose ,blood lipid and biochemical indexes were compared and analyzed among three groups ,and Logistic multi - factor gradual regression analysis was used to perform risk factor analysis .Results : Serum omentin - 1 level in CO + EH group was significantly lower than those of pure CO group and normal control group [ (25.15 ± 3.95) ng/ml vs .(45.63 ± 9.66) ng/ml ,(53.12 ± 7.97) ng/ml , P < 0.05 or < 0.01] ,but there was no significant difference between pure CO and normal control group , P > 0.05 ;Logistic multi - factor gradual regression analysis indicated that age and homeostasis model - insulin resistance index (HOMA - IR ) were independent risk factors affecting EH occurrence in obese people (OR = 1.124 ,95% CI : 1.000 - 1.248 ,P = 0.049 ;OR = 3.446 ,95% CI : 1.087 - 5.607 ,P = 0.001) ,while serum omentin - 1 level was an independent protective factor (OR = 0.423 ,95% CI : 0.210 - 0.636 , P = 0.001) .Conclusion : Serum omentin - 1 level may possess certain guiding significance in early diagnosis ,prevention and treatment for patients with central obesity complicated essential hypertension .%目的：探讨血清网膜素－1在中心肥胖（CO）伴原发高血压（EH）患者的表达及其意义。方法：采用酶联免疫吸附试验检测57例无原发高血压病中心肥胖者（单纯CO组）和67例CO伴EH患者（CO＋ EH组）血清网膜素－1水平，并选56名正常健康者作为正常对照组。对各组血糖、血脂生化指标进行比较分析，并利用Logistic多元逐步回归进行危险因素分析。结果：CO ＋ EH 组血清网膜素－1水平明显低于单纯 CO 组和正常对照组[（25.15±3.95） ng／ml比（45.63±9.66） ng／ml、（53.12±7.97） ng／ml ， P＜0.05或＜0.01]，单纯CO组与正常对照组间血清网膜素－1水平无统计学差异（P＞0.05）；Logistic多元逐步回归分析表明年龄和稳态模型胰岛素抵抗指数（HOM A －IR）是影响肥胖人群高血压发生的独立危险因素（OR＝1.124，95％ CI：1.000－1.248， P＝0.049；OR＝3.446，95％ CI：1.087－5.607， P＝0.001），而血清网膜素－1水平是独立保护因素（OR＝0.423，95％ CI：0.210－0.636， P＝0.001）。结论：血清网膜素－1水平对中心性肥胖伴原发高血压的早期诊断和防治可能具有一定的指导意义。
