针灸治疗脑梗死后痉挛性瘫痪临床选穴规律研究

摘要

Objective: To analyze the rule of selecting points in acupuncture treatment of spastic paralysis after cerebral infarction based on data mining. Methods: Through retrieved CNKI, Wanfang, and Weipu Database on acupuncture treatment of spastic paralysis after cerebral infarction from 1957 to 2017, related 43 papers were selected, to analyze the rule selecting points in acupuncture treatment of spastic paralysis after cerebral infarction. Results: According to analysis Hegu (LI4), Neiguan (PC6), Sanyinjiao (SP6), Waiguan (SJ5), Jianyu (LI15), Quchi (LI11), Shousanli (LI10), Yanglingquan (GB34), Chize (LU5), and Zusanli (ST36) are the most applied points, which belonging to Large intestine meridian of hand-Yangming, Gallbladder meridian of foot-Shaoyang and Sanjiao meridian of hand-Shaoyang, Sites of the selected acupoints most belong to upper limb, and lower limb. Specific point category is chosen mainly five-shu point, crossing point and eight confluence point. The rule of selecting points in acupuncture treatment of spastic paralysis after cerebral infarction are mainly include: selecting the points of corresponding meridian, local selection, and Attention to specific points. Conclusion: Clinical acupuncture treatment of cerebral infarction after spastic paralysis should be based on the theory of Chinese medicine yin and yang balance, the principle of syndrome differentiation and treatment under the guidance of the choice of limb spastic paralysis where the part belongs to Large intestine meridian of hand-Yangming, Gallbladder meridian of foot-Shaoyang and Sanjiao meridian of hand-Shaoyang, adhere to the syndrome differentiation treatment points, pay attention to specific points, so as to play the role of synergy with the selected acupoints to enhance acupuncture treatment of cerebral infarction after spastic paralysis clinical efficacy.%目的:运用数据挖掘技术分析现代针灸文献治疗脑梗死后痉挛性瘫痪的取穴选用规律.方法:采用计算机检索的方式, 对1957～2017年中国期刊全文数据库 (CNKI) 、万方数据知识服务平台和维普数据库中针灸治疗脑梗死后痉挛性瘫痪的相关文献进行检索及梳理, 得到相关文献43篇, 分析现代针灸治疗脑梗死后痉挛性瘫痪的临床选穴规律.结果:通过分析发现, 现代针灸文献治疗脑梗死后痉挛性瘫痪选取腧穴主要为合谷、内关、三阴交、外关、肩髃、曲池、手三里、阳陵泉、尺泽和足三里, 选取腧穴所属经脉主要为手阳明大肠经、足少阳胆经和手少阳三焦经, 选取腧穴所在部位主要是上肢部和下肢部, 选取腧穴特定穴类别主要是五输穴、交会穴和八脉交会穴.针灸治疗脑梗死后痉挛性瘫痪的取穴规律具有以循经取穴、局部取穴和重视特定穴取穴为主的特点.结论:临床针灸治疗脑梗死后痉挛性瘫痪应在中医阴阳平衡理论、辨证论治原则指导下, 选取以肢体痉挛性瘫痪所在部位属于手阳明大肠经、足少阳胆经和手少阳三焦经为主的腧穴进行配伍, 坚持辨证论治选穴配方, 重视特定穴, 从而发挥所选腧穴协同增效的作用, 增强针灸治疗脑梗死后痉挛性瘫痪的临床疗效.
