中药清浊汤对ⅢB型前列腺炎湿热瘀阻症患者SIgA、IL-8、TNF-α的影响

摘要

目的 通过观察中药清浊汤对ⅢB型前列腺炎(湿热瘀阻症)患者前列腺液免疫球蛋白A(SIgA)、白细胞介素8(IL-8)及肿瘤坏死因子α(TNF-α)的影响,探讨中药清浊汤治疗该疾病的部分作用机制.方法 将60例已确诊为慢性非细菌性前列腺炎患者,中医辩证均为湿热瘀阻型,按随机数字表法分为两组各30例,对照组给予泽桂癃爽胶囊口服治疗,治疗组给予中药清浊汤治疗,两组均以10d为1个疗程,连续治疗3个疗程.观察比较两组患者的临床疗效、国际慢性前列腺炎症状指数评分(NIH-CPSI)和前列腺液SIgA、IL-8、TNF-α的含量.结果 治疗组临床疗效显著优于对照组(P<0.01);治疗后治疗组NIH-CPSI积分明显低于对照组,差异有统计学意义(P<0.05).治疗组患者前列腺液中SIgA降低程度优于对照组(P<0.05),同时IL-8、TNF-α的降低作用也强于对照组(P均<0.05).结论 中药清浊汤对ⅢB型前列腺炎(湿热瘀阻症)患者的前列腺液SIgA具有显著降低能力,同时可以降低IL-8、TNF-α在前列腺液中的表达,这可能是通过增强前列腺局部免疫功能、促进局部代谢而达到治疗目的.%Objective To observe the effects of QingZhuo Decoction on SIgA and IL-8 and TNF-α levels in patients with type ⅢB prostatitis. Methods Sixty patients with type ⅢB prostatitis were randomly divided into the treatment group (n=30) and the control group (n=30). The patients in the treatment group were administrated with QingZhuo decotion,and the patinets in the control group with Zeguilongshuang capluse, a treatment course was 30d. The comprehensive efficacy and SIgA, IL-8, TNF-α levels of the two groups were evaluated after a treatment course. Results Efficiency of the treatment group(86.7%) after three courses of treatment was significantly higher than that of the control group(56.7%), and the difference was statistically significant (P<0.01). NIH-CPSI scores of the treatment group after treatment were significantly lower than those before the treatment and those in control group after treatment, and the differences were statistically significant (P<0.05). SIgA, IL-8, TNF-α levels of the treatment group were significantly lower than that of the control group after treatment, and the differences were statistically significant (P<0.05). Conclusion QingZhuo decotion can significantly downregulated SIgA, IL-8, TNF-α levels. These may be related with improvement in local immunity and metabolism of prostate.
