某医院新生儿住院患者医患纠纷情况调查

Objectives:To analyze causes of hospital-patient dispute and features of patients who are inclined to dispute by investigating the possibility of dispute in several different scenarios.Methods:Family members of inpatients newborns in a top tertiary hospital in Beijing were randomly selected to have a self-made questionnaire.And data were analyzed.Results:Scenario's risk ranges from super high to low.Four scenarios are with super high risk,including patient became worse because of medication error,fracture because of falling down from bed and so on.The highest risk factors of medical dispute are younger,good family financial institute and the 1st baby.61.0％ Respondents would like to buy medical accident insurance.Educational level impacts purchase intention,and family income affects how much they would be willing to pay.Conclusions:Risk of dispute is closely related to both medical malpractice and prognosis of disease.Comprehensive medical inform plays important role on the decrease of the hospital-patient dispute.Acceptance of medical accident insurance has already developed into a high level.%目的:通过调查新生儿患方在不同场景下发生纠纷的可能性,分析引发医患纠纷的原因和容易发生纠纷的患方特征.方法:以北京市某三甲儿童医院新生几住院患者家属为研究对象,随机发放自制调查问卷进行调查,并对调查结果进行统计分析.结果:不同场景引发纠纷的风险不同,给药错误造成病情加重、坠床导致骨折等4种场景具有超高风险;高风险场景中选择纠纷的特征是年龄低、家庭收支状况好、患儿为家中第1个孩子;61.0％的被调查者愿意购买医疗意外险,购买意愿受文化程度影响,愿意支付金额受家庭收入影响.结论:纠纷风险与医疗过失和病情转归紧密相关,充分告知对减少纠纷具有积极意义,医疗意外险接受度较高.
