新型农村合作医疗村级基本药物制度对合作医疗基金的影响分析

摘要

Objective To evaluate the effectiveness of essential drugs policy on the expenditure of the new rural cooperative medical fund in Fangshan district.Methods Residents aged over 40 received free drugs for hypertension, diabetes, coronary heart disease and stroke from local community health service agencies.Expenditure of the cooperative medical fund before and after implement of the new policy was compared.Results All the 18 kinds of essential drugs provided from 2008 t0 2010 were for cardiovascular diseases , accounting for 2％ ( 8.628 million yuan ) of the total expenditure.A total of 20.39 million yuan was saved compared with purchasing from the market.A total of 2.874 million yuan was saved from compensation for medical expenditure.Besides, 0.59 million yuan was saved by the reducing in - patient rate.Conclusion The implement of essential drugs policy can reduce the expenditure of the cooperative medical fund and it was important for the operation of the scheme and the management of chronic diseases.%目的 了解村级基本药物制度对新型农村合作医疗基金的影响,为寻找高效节省的基金运作方式提供依据.方法 将北京市房山区40岁以上参合居民中患有高血压、糖尿病、冠心病或脑卒中且需要药物治疗者纳入村级基本药物干预范畴.计算自试行该政策后,合作医疗基金在采购药品和减少自购药与住院报销方面的节约情况.结果 2008-2010年,村级基本药物制度共发放药品18种,花费合作医疗基金862.8万元,占合作医疗总基金的2%,较市场采购金额节约2 039万元.若按个人自购药物再通过合作医疗报销计算,可节约287.4万元.在率先试行村级基本药物制度的六个乡镇,2009-2010年四种慢性病患者住院减少,可节约合作医疗报销金额59万元.结论 村级基本药物制度可提高合作医疗基金的使用效率,对保障合作医疗基金的平稳运行和控制慢性病流行具有重要意义.
