盆底功能康复治疗产后压力性尿失禁的研究观察

摘要

Objective:To evaluation the curative effect and prognosis of electrical stimulation,pelvic floor muscle biofeedback and electrical stimulation, pelvic floor biofeedback,scene of biological feedback therapy in the treatment of postpartum stress urinary incontinence.Method:120 cases of natural childbirth primipara in author’s Hospital from March 2012 to October 2012 who were found with postpartum stress urinary incontinence postpartum 5-6 weeks,were divided into observation group and control group,60 cases in each group.The control group was given electrical stimulation and pelvic floor biofeedback therapy,the observation group was given electrical stimulation,pelvic floor biofeedback therapy and scene of biological feedback therapy.At 6 months postpartum follow-up,assessed by pad test measured and pelvic floor electrical physiological index in two groups.Result:The pelvic floor electrical measurement of physiological indexes after treatment of two groups were improved obviously,the treatment of urinary incontinence efficiency was no significant difference between the two groups(P>0.05).But 6 months postpartum urinary incontinence recrudesce rate of observation group were decrease than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Biofeedback electrical stimulation combined with pelvic floor muscle training in early postpartum is significant recovery of postpartum pelvic floor muscle function.Choosing differences of treatment options according to the individuality has a good effect to consolidate curative effect and prevent recrudesce.%目的：评价电刺激+盆底肌生物反馈治疗及电刺激+盆底肌生物反馈+场景生物反馈治疗两种盆底功能康复治疗模式对产后压力性尿失禁的疗效观察及预后的影响。方法：选取2012年3-10月笔者所在医院自然分娩、产后5~6周复诊时发现压力性尿失禁的产妇120例，随机分为观察组和对照组，各60例。对照组给予电刺激+盆底肌生物反馈治疗，观察组给予电刺激+盆底肌生物反馈+场景生物反馈治疗。于产后6个月随访，通过两组盆底肌电生理指标测定及尿垫试验进行评估。结果：两组经过治疗后盆底肌电生理指标测定都有明显的提高，治疗尿失禁有效率两组比较差异无统计学意义(P>0.05)。但产后6个月随访尿失禁复发率观察组较对照组明显低，差异有统计学意义(P<0.05)。结论：产后早期进行生物反馈电刺激治疗结合盆底肌训练对产后盆底肌功能的康复有显著意义，根据个体差异选择不同的治疗方案，对巩固疗效，预防复发有良好的作用。
