不同手术方法治疗老年患者股骨粗隆间骨折的临床效果对比

摘要

目的：比较3种不同手术方法治疗老年股骨粗隆间骨折的临床治疗效果。方法：收治手术治疗老年股骨粗隆间骨折患者105例，其中选择动力髋螺钉内固定术为DHS组，股骨近端抗旋髓内钉内固定术为PFNA组，股骨近端锁定加压接骨板内固定术为PFLCP组。结果：PFNA组和PFLCP组手术时间、出血量、负重时间、住院时间及术后并发症均显著优于DHS组(P＜0.05)；在术后1个月PFNA组Harris评分显著高于其他两组(P＜0.05)。结论：老年患者股骨粗隆间骨折手术选择，股骨近端锁定加压接骨板内固定术、经股骨近端抗旋髓内钉内固定术较动力髋螺钉内固定术具有创伤小、术后可早期恢复、并发症少等特点，股骨近端抗旋髓内钉内固定术术后可更早地恢复髋关节功能，临床治疗中应首选此术式。%Objective:To compare the clinical effect of 3 kinds of surgical methods in the treatment of elderly patients with intertrochanteric fractures.Methods:105 elderly patients with intertrochanteric fractures were selected.Patients with the treatment of dynamic hip screw internal fixation as the DHS group;patients taken proximal femoral anti rotation intramedullary nail internal fixation as the PFNA group;the other patients taken proximal femoral locking compression plate internal fixation as the PFLCP group.Results:The operation time,bleeding volume,load time,hospitalization time and postoperative complications of patients in the PFNA group and the PFLCP groups were significantly better than in the DHS group(P<0.05).After treated for 1 month,the Harris score in the PFNA group was significantly higher than that in the other two groups(P<0.05).Conclusion:Compared with the surgery of dynamic hip screw internal fixation,proximal femoral locking compression plate internal fixation and the anti rotation intramedullary nail internal fixation in the treatment of elderly patients with femoral intertrochanteric fracture has characteristics of less trauma,early recovery,less complications and others.The hip joint function of patients can recovered earlier with the proximal femur is anti rotation intramedullary nail internal fixation,so this sugery method should be preferred in the clinical treatment.
