Bmi-1与乳腺癌发生、发展及预后的相关性研究

             

摘要

目的 研究Bmi-1在乳腺癌发生、发展中的作用及其预后价值.方法 采用免疫组化En Vision法检测38例正常和普通增生乳腺组织、41例不典型增生乳腺组织、33例乳腺原位癌及129例乳腺癌组织中Bmi-1的表达情况,对Bmi-1与乳腺癌的临床分期、腋窝淋巴结转移、激素受体、HER-2、月经、病理类型、无病生存时间以及总生存时间的关系进行统计学分析.结果 Bmi-1在乳腺癌中的表达高于其在原位癌、不典型增生、正常和普通增生组织中的表达,差异有统计学意义(P<0.05).Bmi-1在原位癌、不典型增生组织中的表达差异无显著性,但均高于在正常和普通增生组织中的表达,差异有统计学意义(P<0.05).Bmi-1与乳腺癌的临床分期、激素受体、HER-2、月经、腋窝淋巴结转移及病理类型无关(P>0.05);Bmi-1低表达患者的无疾病进展生存时间优于高表达者,但二者总生存时间差异无统计学意义.多因素回归分析表明,Bmi-1不是乳腺癌预后的独立因素.结论 Bmi-1参与了早期乳腺癌的发生、发展,且低表达患者的无病生存时间长于Bmi-1高表达者,其对乳腺癌患者的预后价值有待进一步的研究.%Objective To investigate the role of Bmi-I in breast cancer tumorigenesis and development and its prognostic value. Methods Expression of Brai-1 was detected in 38 samples of normal or common hyperplasia breast tissue (3 cases of normal breast tissue and 35 cases of common hyperplasia breast tissue), 41 samples of atypical hyperplasia breast tissue, 33 samples of breast carcinoma in sim (28 cases of intraductal carcinoma and 5 cases of carcinoma in situ) and 129 samples of invasive breast cancer with immunohisto-chemistry EnVision method. The relationships among Bmi-I expression and clinic stage, hormone receptor status, menstruation status, axillary lymph node metastasis status, pathological type, HER-2 status, disease free survival and overall survival were also investigated. Results Expression of Bmi-1 in invasive breast cancer was stronger than that in breast carcinoma in sim,atypical hyperplasia breast tissue, normal or common hyperplasia breast tissue,showing statistical significance( P <0. 05 ) ;there was not significant difference between the Bmi-I expression in breast carcinoma in sim and that in atypical hyperplasia breast tissue, but both expression was stronger than that in normal or common hyperplasia breast tissue with significant difference( P <0.05 ) ;there were not relationships among the Bmi-1 expression and clinical stage, hormone receptor status, menstruation status, axillary lymph node metastasis status or pathological type (P>0.05); the disease free survival of those patients with low Bmi-1 expression was better than those with stronger Bmi-l expression,but there was not significant difference in the overall survival. Multivariate analysis utilizing Cox regression analysis showed that Bmi-I was not an independent prognosis biomarker. Conclusion Bmi-1 plays an important role in breast cancer tumorigenesis and development, and more researches should be done to identify its value for prognosis.
