单纯修补与胃大部分切除术治疗急性胃穿孔的疗效对比分析

摘要

目的：探讨胃大部切除与单纯修补术治疗急性胃穿孔的疗效比较。方法：选择2012年6月－2014年6月北京市怀柔区中医医院就诊的急性胃穿孔患者64例，分为缝合组与切除组，比较两组患者术后各指标，评价两种手术的疗效。结果：切除组在手术时间、住院时间、切口感染率均高于缝合组（P 值分别为：＜0．01，＜0．01，0．01），复发率两组无明显差异（P ＝0．689），缝合组在复发时间上要短于切除组，且差异有统计学意义（P ＝0．018）。结论：单纯修补术治疗急性胃穿孔相比传统胃大部切除术可明显缩短手术时间和住院时间，并减少切口感染的发生，值得在临床上推广应用。%To investigate the comparative effectiveness of subtotal gastrectomy and simple closure for the treatment of acute gastric perforation.Methods:Chose 64 cases acute gastric perforation in T.C.MHospital of Beijing Huairou in 2012.7 -2014.6 which divided into the suture group and resection group.Compare indexes of the two groups after operation,and evaluate the effect of two kinds of opera-tion.Results:Resection group is higher than suture group in operation time,hospitalization time,and incision infection rate (P:<0.01,<0.01,0.01 respectively),there is no significant difference concerning the recurrence rate between the two groups (P =0.689).In ad-dition,suture group is shorter than resection group regarding the time to recurrence,and there is a significant statistically difference (P =0.018).Conclusion:Compared with the traditional subtotal gastrectomy,simple suture technique for the treatment of acute gastric perfora-tion can obviously shorten the operation time and hospital stay,and reduce the occurrence of incision infection.In a word,it is worth popu-larizing and applying on clinic.
