目的：研究对比脊柱骨质疏松性压缩性骨折患者采用经皮椎体成形术(PVP)与经皮椎体后凸成形术(PKP)的临床效果。方法96例(112个椎体)脊柱骨质疏松性压缩性骨折患者,按随机数字表法分为PVP组与PKP组,各48例(56个椎体),均给予相应治疗,观察治疗效果。结果术后PKP组患者前椎、后椎高度分别为(83.6±10.3)、(85.4±11.7)mm,明显高于PVP组(75.9±9.2)、(78.3±10.5)mm (P<0.05)；PKP组骨水泥渗漏率30.4%低于PVP组60.7%(P<0.05)；术后PKP组与PVP组患者疼痛程度评分[(1.1±0.5)、(1.2±0.6)分]及活动能力评分[(1.2±0.2)、(1.3±0.3)分]相比术前疼痛程度评分[(2.4±1.5)、(2.2±1.6)分]及活动能力评分[(2.4±0.6)、(2.3±0.4)分]改善(P<0.05),但两组间比较差异无统计学意义(P>0.05)。结论 PKP与PVP治疗脊柱骨质疏松性压缩性骨折,均能明显缓解患者的疼痛, PKP术后骨水泥渗漏率更低,恢复椎体高度更佳。%Objective To research and compare clinical effects between percutaneous vertebroplasty (PVP) and percutaneous kyphoplasty (PKP) in the treatment of osteoporotic vertebral compression fractures patients. Methods A total of 96 osteoporotic vertebral compression fractures patients (112 centrum) were divided by random number table into PVP group and PKP group, with 48 cases (56 centrum) in each group. Both groups received corresponding treatment, and their curative effects were observed.Results PKP group had obviously higher postoperative anterior and posterior vertebral height as (83.6±10.3) and (85.4±11.7) mm than (75.9±9.2) and (78.3±10.5) mm in PVP group (P<0.05). PKP group had lower bone cement leakage rate as 30.4% than 60.7% of PVP group (P<0.05). Both groups had better pain degree score [(1.1±0.5) and (1.2±0.6) points] and activity ability score [(1.2±0.2) and (1.3±0.3) points] after operation than pain degree score [(2.4±1.5) and (2.2±1.6) points] and activity ability score [(2.4±0.6) and (2.3±0.4) points] before operation (P<0.05), while the difference between the two groups had no statistical significance (P>0.05).Conclusion Both PKP and PVP provide remarkably pain relief in treating osteoporotic vertebral compression fractures, while PKP shows lower postoperative bone cement leakage rate and better vertebral height recovery.
