甲钴胺与维生素B1联合维生素B12治疗视神经炎的疗效比较

ObjectiveTo observe and compare clinical effects by mecobalamine and vitamin B1 combined with vitamin B12 in the treatment of optic neuritis.MethodsA total of 78 optic neuritis patients (99 eyes) were randomly divided into mecobalamine treatment group with 38 cases (47 eyes, treatment group) and vitamin B1 combined with vitamin B12 treatment group with 40 cases (52 eyes, control group). Both groups received other treatment ways as the same. Their clinical effect after 1 month, examination result of visual evoked potential (VEP) and perimetry were taken for statistical analysis.ResultsAfter 1 month of treatment, the treatment group showed better clinical effect than the control group (P<0.05). The treatment group had higher P100 amplitude in VEP as (7.58±3.52)μV than (6.79±3.17)μV of the control group (P<0.05). The treatment group had lower incubation period as (91.56±13.91) ms than (97.18±16.55) ms of the control group (P<0.01). Perimetry examination showed that the treatment group had higher average light sensitivity as (24.85±4.07) dB than (22.14±3.68) dB of the control group (P<0.01). Average visual field defect was lower in the treatment group as (0.54±0.31) dB than (0.63±0.27) dB in the control group (P<0.01).ConclusionAs a safe and effective drug in treating optic neuritis, mecobalamine provides slight adverse reactions. Its clinical effect is better than combination of vitamin B1 and vitamin B12. This method is easy for application in primary hospital, and it is worth further promotion.%目的：观察比较甲钴胺与维生素B1联合维生素B12治疗视神经炎的临床疗效。方法78例(99眼)视神经炎患者随机分为甲钴胺治疗组38例(47眼,治疗组)和维生素B1联合维生素B12治疗组40例(52眼,对照组),两组其他治疗方法相同,对两组治疗1个月后临床疗效、视觉诱发电位(VEP)检查及视野检查结果进行统计分析。结果治疗1个月后,治疗组临床疗效优于对照组(P<0.05)；治疗组VEP检查显示P100波振幅为(7.58±3.52)μV,高于对照组的(6.79±3.17)μV(P<0.05),治疗组潜伏期(91.56±13.91)ms,低于对照组的(97.18±16.55)ms(P<0.01)；治疗组视野检查结果显示平均光敏度为(24.85±4.07)dB,高于对照组的(22.14±3.68)dB(P<0.01),治疗组平均视野缺损(0.54±0.31)dB,低于对照组的(0.63±0.27)dB(P<0.01)。结论甲钴胺是治疗视神经炎安全有效的药物,不良反应轻微,临床疗效优于维生素B1联合维生素B12,该药用法简单,基层医院均可实施,值得进一步推广。
