Objective: To study the property of outpatient prescriptions in a grade A class three teaching hospital and take effective intervention to reduce irrational drug utilization. Method: A total of 20 400 outpatient prescriptions were screened randomly during A-pril 2010 to January 2011. Totally 200 prescriptions were withdrawn every two months in each department. The prescriptions were evaluated by pharmacists according to "Administrative Rules on Prescriptions" and "Review on the Management Provisions of Hospital Prescription". The evaluation contents included the essential elements of prescriptions, dosage, usage duration, indications and drug interactions. Result: The average drug number in the prescriptions was 1. 84. The percentage of antibiotics was 24. 95% . 89. 95% prescriptions were rational. The percentage of irrational prescriptions were decreased gradually from 19. 71% to 5. 88% after one-year prescription evaluation and outpatient education( P 和实时进行处方点评,点评内容包括处方的基本要素、处方用药剂量、使用疗程、适应证、药物相互作用等信息.结果:每张处方用药品种为1.84个,抗菌药使用率为24.95%,合理处方百分率为89.95%.经过一年的点评并采取干预措施后,不合理处方从19.71%下降到5.88%(P<0.005).结论:处方点评和干预措施明显的改善了处方质量.加强临床药师的药学专业和临床专业知识学习将会进一步提高不合理处方的发现率.
