目的：对静脉用药调配中心不合理医嘱进行统计分析和比较，探讨医嘱审核及干预、培训及反馈、再干预三步走的措施对药学干预的效果。方法通过边干预边收集数据的方式，对2013年前3个季度不合理医嘱进行统计和分析。结果经过有效的干预，不合理医嘱比例明显下降。结论特有的干预与培训反馈交叉配合的手段可以有效降低不合理医嘱比例，同时，积极的沟通和学习建立了药师和医师之间交流的良性循环，保证了药学干预的效果。%Objectiv To analyze and compare the irrational medical orders in Pharmacy Intravenous Admixture Service ( PIVAS ) , and to discuss the effect of the three-step measures ( check and intervention, training and feedback, re-intervention ) on the pharmaceutical intervention. Methods The simultaneous intervention and data collection were adopted to conduct the statistical analysis on the irra-tional medical orders in the first three quarters of 2013. Results By the effective interventions, the proportion of irrational medical orders was significantly declined. Conclusion The specific cross coordinating measures of intervention with training and feedback can effectively reduce the proportion of irrational medical orders, at the same time the active communication and learning establish a vir-tuous circle between pharmacists and physicians, which ensures the effectiveness of the pharmaceutical intervention.
