目的：了解万古霉素在老年住院患者中的临床应用情况，评价其用药合理性，为临床合理用药提供参考依据。方法对解放军总医院2011年11月至2012年4月使用万古霉素的286例老年住院患者进行回顾性调查分析。结果286例患者中，66例治疗用药，220例手术预防用药。66例治疗用药中，治疗有效率为72.73%；220例手术预防用药中，手术预防有效率为95.00%。286例使用万古霉素的患者中，不良反应发生率为2.45%。286例患者中，用药合理和基本合理共194例（67.83%），不合理用药92例（32.17%）。结论该院老年住院患者应用万古霉素的情况基本合理，但用药的病原学检查还有待加强。%Objective To understand the use situation of vancomycin in elderly inpatients and to evaluate the rationality of its use to provide reference for its rational use in clinic. Methods A total of 286 elderly inpatients using vancomycins in our hospital from November 2011 to April 2012 were retrospectively investigated and analyzed. Results Among 286 cases,66 cases used vancomycin in therapy with the curative effect rate of 72. 73% and 220 cases used vancomycin as the operative prevention medication with the pre-ventive effect rate of 95. 00% . Among the 286 cases of using vancmycin,the incidence rate of adverse reactions was 2. 45% . Of all the 286 cases,the use of vancomycin was basically rational in 194 cases(67. 83% ) and irrational in 92 cases(32. 17% ). Conclusion The application of vancomycins in elderly inpaients in our hospital is basically rational,but the pathogenic detection remains to be strengthened.
