美多巴与多种药物联合治疗早期非创伤性股骨头坏死56例

         

摘要

目的:探讨美多巴与多种药物联合治疗早期非创伤性股骨头坏死患者骨密度的临床效果。方法将2013年3月至2014年3月收治的56例非创伤性股骨头坏死患者作为研究对象,所有患者通过美多巴和抗凝药、双膦酸盐及中药联合治疗,并在治疗前后接受双髋前后位的磁共振成像和X线摄片检查,统计患者的检查结果。结果 X线摄片检查早期非创伤性股骨头坏死患者治疗后股骨头的骨密度较治疗前明显增大;治疗后的坏死指数为(49.54±30.08)分,比治疗前下降显著( P<0.05);塌陷率为30.09%;临床改善率61.53%;治疗成功率69.45%;治疗后患者Merle D'Aubigne评分总分(16.21±2.32)分,显著高于治疗前的(13.16±2.71)分( P<0.05)。结论美多巴联合抗凝药、双膦酸盐及中药治疗早期股骨头坏死患者的临床效果十分明显,可以促进骨形成,加速骨愈合修复,降低股骨头塌陷率。%Objective To explore the clinical effect of madopar combined with multiple drugs on the bone mineral density ( BMD ) for treating early non-traumatic femoral head necrosis. Methods Totally 56 patients with early non-traumatic femoral head necrosis in our hospital from March 2013 to March 2014 were chosen as the research subjects and performed the combined therapy of oral madopar anticoagulation, diphosphonate and traditional Chinese medicines. And the magnetic resonance imaging ( MRI ) and plain radio-graphs were conducted before and after treatment. All examination results were collected and analyzed for evaluating the clinical treat-ment effect by scores. Results BMD of femoral head after treatment was significantly increased compared with before treatment, the necrosis index after treatment was ( 49. 54 ± 30. 08 ) , which was significantly decreased compared with before treatment( P < 0. 05);the collapse rate was 30. 09%, the clinical improvement rate was 61. 53% and the treatment success rate was 69. 54%, the Merle D’Aubigne scores after treatment reached ( 16. 21 ± 2. 32 ) points, which was significantly higher than ( 13. 16 ± 2. 71 ) points before treatment, which fully indicated that this combined therapy effectively improved the lesion symptoms, relieved the disease condition and the difference was statistically significant ( P < 0. 05 ) . Conclusion Madopar combined with multiple drugs for treating early non-traumatic femoral head necrosis has very obvious effect, can promote the bone formation, accelerate the repair of bone healing, reduce the femoral head collapse rate and is worthy of being clinically popularized and applied.
