心脉隆注射液联合阿托伐他汀钙治疗慢性肺源性心脏病并右心衰竭50例临床观察

摘要

目的 观察心脉隆注射液联合阿托伐他汀钙治疗慢性肺源性心脏病并右心衰竭的效果.方法 选取2016年5月至2017年2月收治的慢性肺源性心脏病并右心衰竭患者100例,随机分为对照组和观察组,各50例.对照组患者予阿托伐他汀钙治疗,观察组患者在此基础上加用心脉隆注射液静脉滴注,治疗15 d.结果 治疗后,两组患者的右室射血分数(RVEF)、B型钠尿肽(NT-proBNP)及C反应蛋白(CRP)均较治疗前明显改善(P<0.01),且观察组的改善情况显著优于对照组(P<0.01);治疗后,两组患者的动脉血氧饱和度(SaO2)、血氧分压(PaO2)、二氧化碳分压(PaCO2)均明显好转(P<0.01),且观察组改善情况均显著优于对照组(P<0.01);治疗后,两组患者的肺动脉收缩压(PASP)及肺动脉舒张压(PADP)均较治疗前显著下降(P<0.01),且观察组显著低于对照组(P<0.01);观察组总有效率为92.00％,显著高于对照组的74.00％(P=0.017).结论 心脉隆注射液联合阿托伐他汀钙治疗慢性肺源性心脏病并右心衰竭,能有效地降低患者的肺动脉压力,改善心功能,值得临床推广.%Objective To investigate the clinical effect of Xinmailong Injection combined with atorvastatin calcium in the treatment of chronic cor pulmonale complicated with right heart failure. Methods Totally 100 patients with chronic cor pulmonale complicated with right heart failure admitted to our hospital from May 2016 to February 2017 were selected and randomly divided into the control group and the observation group,50 cases in each group. The control group was treated with atorvastatin calcium,on this basis,the observation group was treated with Xinmailong Injection. The two groups were treated for 15 d. Results After treatment,the levels of RVEF,NT-proBNP and CRP in the two groups were significantly improved compared with those before treatment(P < 0. 01),and the improvement in the observation group was significantly better than that in the control group(P < 0. 01). After treatment,the SaO2,PaO2 and PaCO2 of the two groups were significantly improved(P < 0. 01),and the improvement in the observation group was significantly better than that in the control group(P < 0. 01). After treatment,the PASP and PADP of the two groups were significantly lower than those before treatment(P < 0. 01),and those in the observation group was significantly lower than those in the control group(P < 0. 01). The total effective rate of the observation group was 92. 00％,which was significantly higher than 74. 00％ of the control group(P=0. 017). Conclusion Xinmailong Injection combined with atorvastatin calcium in the treatment of chronic cor pulmonale complicated with right heart failure can effectively reduce the pressure of the pulmonary artery and improve the heart function,which is worthy of clinical promotion.
