基层医院自然分娩后未使用抗生素的效果观察

摘要

Objective To explore the effect of no antibiotic using after natural childbirth in primary hospital. Methods 486 cases of natural labor puerpera withⅠ-II degree perineal laceration or episiotomy from January 2012 to Decem-ber 2013 were selected and randomly divided into two groups,the experimental group was given postpartum treatment according to clinical path,not given antibiotics treatment,the control group was given antibiotic treatment after child-birth.The maternal perineal wound infection rate,wound healing rate,puerperal morbidity,the average cost of hospitaliza-tion of the two groups were compared. Results The maternal perineal wound infection rate,wound healing rate,puer-peral morbidity of two groups compared,there was no significant difference(P>0.05),the average delivery cost of experi-mental group was significantly lower than that of the control group,with statistical difference(P<0.05). Conclusion Antibi-otic using or not has no significant influence on wound healing and puerperal morbidity of natural labor vaginal deliv-ery puerpera with Ⅰ-II degree perineal laceration or episiotomy in primary hospital,so postpartum routine use of an-tibiotic for infection preventing is not required,while it can reduce hospitalization costs,it is worthy of clinical promotion and application.%目的：探讨基层医院自然分娩后未使用抗生素的效果。方法选择2012年1月~2013年12月本院收治的486例自然分娩合并会阴Ⅰ~Ⅱ度裂伤或实施会阴切开的产妇作为研究对象，随机分为两组，试验组按临床路径处理，未进行抗生素治疗，对照组则给予抗生素治疗，比较两组的会阴伤口感染率、伤口愈合率、产褥病发生率、平均分娩费用。结果两组的会阴伤口愈合率、产褥病发生率比较，差异无统计学意义(P>0.05)；试验组的平均分娩费用显著低于对照组，差异有统计学意义(P<0.05)。结论基层医院自然分娩合并会阴Ⅰ~Ⅱ度裂伤或实施会阴切开的产妇是否使用抗生素对伤口愈合及产褥病发生率并无明显影响，因此产后不需常规使用抗生素预防感染，这样可降低医疗费用，值得临床推广应用。
