目的：探讨泼尼松联合吗替麦考酚酯对IgA肾病患者血清中血管内皮生长因子（VEGF）和基质金属蛋白酶-9（MMP-9）的影响。方法选取2012年6月~2014年6月在本院接受治疗的102例IgA肾病患者作为研究对象，将其按照随机、自愿的原则分为对照组和观察组，每组51例。对照组患者在常规治疗基础上加用泼尼松治疗，观察组在对照组基础上采用吗替麦考酚酯治疗，两组疗程均为6个月。比较两组患者的临床疗效、血清VEGF和MMP-9的变化情况。结果观察组患者的显效率为84.3%，显著高于对照组的54.9%，差异有统计学意义（χ²=5.653，P=0.024）。治疗前两组血清VEGF和MMP-9水平差异均无统计学意义（P>0.05）；治疗后两组血清VEGF和MMP-9水平均显著低于治疗前，差异有统计学意义（均P<0.05），且观察组较对照组下降更明显，差异有统计学意义（P<0.05）。结论泼尼松联合吗替麦考酚酯能够有效提高IgA肾病患者的治疗效果，可能与下调血清VEGF和MMP-9水平的表达有关。%Objective To discuss the influence of prednisone combined with mycophenolate mofetil on serum vascular endothelial growth factor (VEGF) and matrix metalloproteinase-9 (MMP-9) in patients with IgA nephropathy. Methods 102 patients with IgA nephropathy admitted into our hospital from June 2012 to June 2014 were selected as research object,they were randomly and voluntarily divided into control group (n=51) and observation group (n=51).The control group was given prednisone treatment on basis of conventional therapy,the observation group was given mycophenolate mofetil based on treatment of the control group,the course was 6 months.The clinical effects,serum concentration of VEGF and MMP-9 between the two groups were compared before and after treatment. Results The significant efficien-cy of observation group was 84.3%,which was significantly higher than 54.9% of control group,the difference was signif-icant (χ²=5.653,P=0.024).There was no statistical significance about serum concentration of VEGF and MMP-9 at be-fore treatment (P>0.05).After treatment,the serum concentration of VEGF and MMP-9 of two groups was significantly lower than before treatment,and observation group than the control group fell more obvious (P<0.05).Conclusion The treatment of prednisone combined with mycophenolate mofetil can effectively enhance the clinical effect in patients with IgA nephropathy,it is maybe related to reduce the expression of serum VEGF and MMP-9.
