带内导芯气囊导尿管耻骨上膀胱穿刺造瘘与套管针穿刺造瘘方法的比较

摘要

Objective To evaluate the merit of a method of suprapubic puncture cystostomy with balloon catheter of indwelling guide wire.Methods We divided 110 patients who need cystostomy into experimental group and control group.In experimental group we introduce suprapubic puncture cystostomy with balloon catheter of indwelling guide wire,and in control group we using trocar to complete suprapubic puncture cystostomy.Puncture time,duration of hematuria after cystostomy and cage of urine leakage are selected as measurable indicators.Results The operation was successful in all cases of two groups.The puncture time was(7±0.3)min in control group vs (4±0.5)min in experimental group,with significant differences between the two groups(P＜0.01).The duration of hematuria was(3±0.5)day in control group vs(1±0.6)day in experimental group,cases of urine leakage in control group are 40 vs 16 in experimental group.both with significant differences between the two groups(P＜0.05).Conclusion Our method is superior to the traditional suprapubic bladder cyatostomy,and not only reduce the suffering of patients,but also is safe and convenient.%目的 评估带内导芯气囊导尿管耻骨上膀胱穿刺造瘘方法 的优缺点.方法 1120例膀胱造瘘患者采用完全随机方法 分为实验组588例和对照组532例,对照组采用套管针耻骨上膀胱穿刺造瘘方法 ,实验组采用带内导芯气囊导尿管耻骨上膀胱穿刺造瘘法.对2组穿刺时间、术后血尿持续时间和漏尿例数作为衡量指标进行对比.结果 实验组所需穿刺时间明显少于对照组,采用两样本均数t检验,差异具有统计学意义(P＜0.01);术后血尿持续时间短于对照组,2组间采用t检验,差异具有统计学意义(P＜0.01);漏尿发生率低于对照组,漏尿例数做构成比X2检验,差异有统计学意义(P＜0.05).结论 带内导芯气囊导尿管耻骨上膀胱穿刺造瘘方法 优于套管针耻骨上膀胱穿刺造瘘,不仅可减轻患者痛苦,而且安全方便.
