LEEP刀治疗宫颈CIN病变临床分析

摘要

目的：观察LEEP刀宫颈切除术对宫颈CIN病变疗效的影响，为HPV引起的CIN病变的临床治疗进一步提供必要的临床依据。方法检测本院2010年1月～2012年12月门诊患者，PCR方法检测患者HPV感染情况，选择68例HPV阳性患者行宫颈活检，对病理学诊断CINI-III级患者行LEEP刀宫颈切除术。并在术后12个月内和24个月内行宫颈细胞学TCT和宫颈HPV检查，对LEEP刀宫颈切除术的疗效进行评价。结果（1）Leep刀宫颈切除术后宫颈细胞学及HPV检查结果显示0/∑I+II+III比值较手术前明显增高,两组差异存在统计学意义（ P ＜0.05）。（2）LEEP刀宫颈切除术后组观察组1和观察组20/∑I+II+III值比较，两组差异亦存在统计学意义（ P＜0.01）。（3）LEEP刀宫颈切除术后12个月内和24个月内HPV检查结果显示较手术前明显降低，组间差异存在统计学意义（P＜0.05）。结论LEEP刀宫颈切除术对宫颈CIN病变有明显疗效；LEEP刀宫颈切除术对宫颈CIN病变HPV感染率的降低与多种因素有关，LEEP刀宫颈切除术患者要强化HPV感染的控制。%Objective To observe the impact of cervicectomy with LEEP knife on the curative effect of CIN lesions, in order to provide the further necessary clinical basis for the clinical treatment of CIN lesions caused by HVP. Methods The HPV infection of outpatients in our hospital from January 2010 to December 2012 were determined by PCR method. 68 patients with HPV positive were done cervical biopsy,and the patients with pathological diagnosis of CINI-III were done cervicectomy with LEEP knife.The cervical TCT and HPV were done within 12 months and 24 months after the operation, and the effect of cervicectomy with LEEP knife were evaluated. Results (1)The cervical TCT and HPV results showed that the 0/∑I+II+III rate after the cervicectomy with Leep knife increased significantly compared with before the operation, and the difference was statistically significant between the two groups(P<0.05).(2) There was statistically significant difference in the 0/∑I+II+III rate between the observation group 1 and group 2 after the cervicectomy with LEEP knife(P<0.01).(3)The HPV within 12 months and 24 months after the cervicectomy with Leep knife decreased significantly compared with before the operation, and the difference was statistically significant between the two groups(P<0.05). Conclusion The cervicectomy with LEEPknife has obvious curative effect on the CIN lesions; The cervicectomy with LEEP knife on the decrease of HPV infection rate of CIN lesions is correlated with many factors,so the patients with cervicectomy with LEEP knife should be strengthened the control of HPV infection.
