罗哌卡因头皮切口浸润麻醉在开颅手术中的应用研究

         

摘要

目的:研究罗哌卡因混合液头皮切口浸润麻醉在开颅手术中的应用。方法随机选取于2013年6月~2014年10月前来我院进行开颅手术治疗的患者共60例作为本次研究对象。随机将患者分为观察组和对照组,观察组患者切口应用1%罗哌卡因+1∶20万肾上腺素+生理盐水混合液局部浸润,对照组患者应用生理盐水手术切口浸润。结果两组患者的血流动力学变化,在 T3、T4、T5、T6时对照组动脉压(MAP)、心率(HR)、呼气末异氟醚的浓度均明显高于观察组(P <0.05);而对照组拔管时间(31.1±4.9)min,明显长于观察组的(16.1±2.2)min(P <0.05)。结论切口浸润1%罗哌卡因混合液与全身麻醉复合的使用,能有效减轻机体的应激反应,减少全麻药的用量,维持血流动力学的稳定,值得在神经外科麻醉方面广泛推广。%Objective To research the application of ropivacaine scalp incision infiltration anesthesia in craniotomy. Methods 60 patients who were admitted to our hospital and received craniotomy from June 2013 to October 2014 were selected as research subjects. They were randomly allocated to observation group and control group. The incisions of observation group were infiltrated with 1% ropivacaine, 1:200000 adrenaline and normal saline, and the incisions of control group were infiltrated with normal saline. Results Hemodynamic changes of two groups were compared, and mean artery pressure (MAP), heart rate (HR), and density of end-expiratory isoflurane at T3, T4, T5 and T6 in control group were significantly higher than those in observation group (P < 0.05) . Extubation time in control group was (31.1±4.9) min, which was significantly longer than that of observation group (16.1±2.2) min (P < 0.05). Conclusion Incision infiltrated with 1% ropivacaine mixture combined with general anesthesia can effectively alleviate stress reaction of organism, reduce general anesthetic, and maintain hemodynamic stability, which is worthy of widespread promotion in anesthetic department of neurosurgery.
