目的：探讨股骨远端LISS钢板倒置治疗复杂股骨粗隆间骨折的临床疗效。方法选取本院2012年12月～2014年12月诊治的复杂股骨粗隆间骨折患者46例，采用随机数字表法分为两组，对照组患者23例实施DHS内固定治疗，观察组患者23例实施股骨远端LISS钢板倒置治疗，比较两组患者的治疗效果。结果观察组手术时间、术中出血量、术后引流量、术后住院时间少于对照组（P＜0.05），切口长度小于对照组（P＜0.05），骨折愈合时间、患肢消肿时间早于对照组（P＜0.05），术后Harris评分（疼痛、功能、畸形、运动范围、总分）、术后血红蛋白高于对照组（P＜0.05），术后红细胞沉降率、并发症发生率低于对照组（P＜0.05），患肢肿胀程度轻于对照组（P＜0.05）。结论股骨远端LISS钢板倒置治疗复杂股骨粗隆间骨折的疗效显著，值得临床推广使用。%Objective To explore the clinical effect of the upside down mounting of LISS plate at distal femur in the treatment of complex femoral intertrochanteric fracture.Methods 46 patients with complex femoral intertrochanteric fracture who were in our hospital from December 2012 to December 2014 were randomly divided into two groups,23 cases in control group received the treatments of DHS internal fixation, 23 cases in observation group received the treatments of LISS plate upside down mounting at distal femur, and the treatment effects of two groups were compared. Results In the observation group, compared with the control group, the operation time, blood loss during operation, drainage quantity after operation, and postoperation hospitalization time were much less or shorter (P<0.05),the incision size was much shorter (P<0.05),the fracture healing time and ill limb detumescence time were much shorter (P<0.05),the Harris scores, including pain score,function score,abnormality score,range of movement score, and total score, as well as the postoperative hemoglobin level were much higher (P<0.05),the erythrocyte sedimentation rate and postoperative complication rate were much lower (P<0.05),and the degree of ill limb swelling was much lower (P<0.05).Conclusion The upside down mounting of LISS plate at distal femur has a significant effect in the treatment of complex femoral intertrochanteric fracture, it is worth to be clinical popularized.
