保乳手术与改良根治术治疗早期乳腺癌近远期疗效对比

         

摘要

Objective: To explore the recent and long-term effect on breast-conserving therapy and modified radical mastectomy in the treatment of the early-stage breast cancer. Methods: The data of 70 cases of patients with early-stage breast cancer of grade I , II from December 2002 to December 2007 were retrospectively analyzed and divided into BCT group and MRM group according to their surgical method. The curative effects and quality of life were compared. Results: The incidence of subcutaneous fluid, ipsilateral upper extremity edema, flap necrosis in BCT group was lower than that of MRM group, there was a significant difference in the subcutaneous fluid (P<0.05). No patient died due to associated diseases (0), the survival rate of three years was 100.0%, local recurrence after surgery was 4 cases (11.4%), distant metastasis was 3 cases (8.6%); 1 patient died due to associated diseases (2.9%), the survival rate of three years was 97.1%, local recurrence after surgery was 5 cases (14.3%), distant metastasis was 4 cases (11.4%). There was no significant difference between the two groups in survival rate, mortality rate and local recurrence rate (P>0.05). Postoperative cosmetic and quality of life in BCT group were significantly higher than those in MRM group (P<0.001). Conclusion: The recent and long-term effect of BCT for the patients with early-stage breast cancer is effective with few complications and high quality of life, which are better than MRM.%目的:探讨保乳手术和改良根治术治疗早期乳腺癌的近远期疗效.方法:回顾性分析2002 年12 月~2007 年12月我科诊治70 例Ⅰ、Ⅱ期乳腺癌患者的临床资料,根据手术方式分为保乳手术组(BCT 组)和改良根治术组(MRM组),比较两组患者术后疗效和生存质量的差异.结果:BCT 组患者术后皮下积液、患侧上肢水肿和皮瓣坏死发生率低于MRM 组,其中在皮下积液方面比较,差异有统计学意义(P<0.05).BCT 组无患者因相关疾病死亡(0),3 年存活率为100.0%,术后局部复发4 例(11.4%),远处转移3 例(8.6%);MRM 组随访结束时1 例患者死亡(2.9%),3 年存活率为97.1%,术后局部复发5 例(14.3%),远处转移4 例(11.4%).BCT 组和MRM 组患者在死亡率、3 年存活率、局部复发和远处转移方面差异无统计学意义(P>0.05).BCT 组术后随访的美容效果优、良率和生存质量评分均显著高于MRM 组,差异有高度统计学意义(P<0.001).结论:保乳手术治疗早期乳腺癌术后并发症少,患者生存质量高,近远期疗效显著高于改良根治术.
