重组人促红细胞生成素对对比剂诱导的大鼠急性肾损伤的保护作用

Objective To explore the protective effect of recombinant human erythropoietin in the acute kidney injury induced by contrast agent in rats. Methods 40 SD rats were randomly divided into five groups: blank control group, experiment control group, model group, rhEPO 1 group [injection of rhEPO of 3 000 IU/(kg·d) before making model for 3 days], rhEPO 2 group [injection of rhEPO of 600 IU/(kg·d) before making model for 3 days]. Serum creatinine (Scr) and blood urea nitrogen (BUN) were observed before making model, after making model for 48 hours and 72 hours. The pathologic changes were observed. Results Scr in model group after contrast agent injection for 48 hours and 72 hours were all increased over 25% than before. Scr and BUN in model group, rhEPO 1 group and rhEPO 2 group were significantly higher than that in blank control group after contrast agent injection for 48 hours and 72 hours (P < 0.05), Scr and BUN in rhEPO 1 group and rhEPO 2 group were significantly lower than that in model group (P < 0.05), and had less histological damage. Scr and BUN in rhEPO 1 group decreased more significantly than that in rhEPO 2 group (P < 0.05). Conclusion rhEPO has the protective effect on the acute kidney injury induced by contrast agent in rats, and the protective effect of big dosage is superior to small dosage.%目的 探讨重组人促红细胞生成素(rhEPO)对对比剂诱导的大鼠急性肾损伤的保护作用.方法 SD大鼠40只,随机分为空白对照组、实验对照组、模型组、rhEPO 1组[造模前3天注射rhEPO 3 000 IU/(kg·d)]、rhEPO 2组[造模前3天注射rhEPO 600 IU/(kg·d)],观察造模前、造模后48、72 h血清肌酐(Scr)及尿素氮(BUN)的变化,观察肾脏病理标本.结果 模型组注射对比剂后48 h和72 h Scr均较注射前升高大于25%;与空白对照组比较,模型组、rhEPO 1组、rhEPO 2组造影后48、72 h Scr、BUN均明显上升(P < 0.05);与模型组比较,rhEPO 1组、rhEPO 2组造影后48、72 h Scr、BUN均有明显下降(P < 0.05),肾组织损害较模型组明显减轻.与rhEPO 2组比较,rhEPO 1组Scr、BUN降低更明显(P < 0.05).结论 重组促红细胞生成素对对比剂诱导的大鼠急性肾损伤有保护作用,大剂量rhEPO保护作用优于小剂量.
