Objective To evaluate the clinical effects and side effects of Capecitabine metronomic chemotherapy in the treatment of advanced gastric cancer. Methods 32 elder patients with advanced gastric cancer were given Capecitabine metronomic chemotherapy after excluding chemotherapy contraindications. 500 mg of Capecitabine was taken orally twice per day for 4 continuous weeks, followed by 1 week's break as one treatment cycle, until the disease progressed or the patient could not tolerate. The curative effects were evaluated after at least one treatment cycle and the adverse effects were observed at the same time. Results None of patients obtained complete remission (CR). The effective rate of primary lesion was 34.4% and the disease control rate was 65.6%; the liver metastasis effective rate was 25.0%; lymph node metastasis effective rate was 13.6%; peritoneal metastasis effective rate was 16.7% and which had no effect on lung metastasis. The incidence of adverse reactions was 68.8% (22/32), of which leukopenia (9 patients), skin pigmentation (9 patients), pernicious vomiting (8 patients), fatigue (7 patients) and diarrhea (5 patients) were common adverse reactions, thus hand-foot syndrome (2 patients) and stomatitis (2 patients) were rare adverse reactions. All adverse reactions were limited to degree II and below. Conclusion Metronomic Capecitabine chemotherapy shows good curative effects, few side effects and good patient compliance in the treatment of elderly patients with advanced gastric cancer, but the exact curative effects need testificaiton of large sample study.%目的 评价卡培他滨节拍式给药在老年晚期胃癌化疗中的临床疗效及毒副反应.方法 32例老年晚期胃癌患者排除化疗禁忌证后给予卡培他滨节拍式化疗,卡培他滨500 mg,日两次口服,连用4周,休息1周为1个周期,直至疾病进展或患者不能耐受,至少1个周期后评价疗效,同时观察不良反应情况.结果 所有患者均未获CR,其中,原发灶有效率为34.4%,疾病控制率为65.6%,肝脏转移有效率为25.0%,淋巴结转移有效率为13.6%,腹腔转移有效率为16.7%,肺转移无效.各种不同程度不良反应发生率为68.8%(22/32),其中,以白细胞减少(9例)、皮肤色素沉着(9例)、恶性呕吐(8例)、乏力(7例)、腹泻(5例)较为常见,而手足综合征(2例)、口腔炎(2例)较为少见,且所有患者的不良反应均局限于Ⅱ度及以下.结论 节拍式卡培他滨化疗对老年晚期胃癌疗效较好,副作用小,患者服药依从性较好,但确切疗效仍有待大样本研究证明.
