心电图对心尖肥厚型心肌病的诊断价值

摘要

Objective To analyze the electrocardiographic feature of apical hypertrophic cardiomyopathy patients, and assess its diagnostic value.Methods The electrocardiogram data of 56 patients with apical hypertrophic cardiomyopathy (AHCM, observation group) and 56 healthy controls (control group) were analyzed retrospectively.The change of electrocardiogram of the two groups were compared, and precordial leads maximum QRS wave amplitude, ST segment level and T wave morphology and amplitude were observed.Results The electrocardiogram of patients with AHCM showed huge T wave inversion, ST-segment depression and high voltage of left ventricles.The observation group patients showed precordial QRS waves prolonged, QRS wave amplitude change significantly in different leads, all more than normal.Performance in the ST segment, V3-V6 and Ⅱ, Ⅲ and aVF depressed respectively.42 cases had V3 -V6 T-wave inversion, of which 36 cases had a depth of 10-16 mm inverted T wave.Conclusion Electrocardiogram plays an important role in diagnosis of AHCM.Early electrocardiogram examination can enhance the diagnostic rate of patients with cardiomyopathy .%目的 分析心尖肥厚型心肌病患者心电图特点,探讨心电图对心尖肥厚型心肌病的诊断价值.方法 对武威市人民医院56例心尖肥厚型心肌病患者(观察组)和同期进行健康体检者56名受试者(对照组)的心电图资料进行回顾性分析,比较两组人群心电图的变化情况,观察胸前导联最大QRS波群振幅、各导联ST段水平、T波形态和振幅.结果 观察组患者心电图主要表现为胸前导联巨大的T波倒置并伴ST段下移以及左室高电压现象;心前导联QRS波群时间延长,且QRS波群振幅在不同导联上变化明显,都超过正常值;在ST段上,分别表现在V3~V6和Ⅱ、Ⅲ、aVF下移;且有42例V3~V6 T波倒置,其中,36例倒置T波深达10~16 mm.结论 心电图对诊断心尖肥厚型心肌病具有重要的临床价值,对有心肌病的患者及早行心电图检查可提高本疾病的诊断率.
