TNF-α基因启动子区多态性与中国人群冠心病易感性关系的Meta分析

摘要

目的 采用Meta分析的方法 探讨肿瘤坏死因子-α(TNF-α)基因启动子区-238G/A、-308G/A和-863C/A多态性与中国人群冠心病易感性的关系.方法 利用PubMed、万方和重庆维普中文科技期刊数据库检索所有相关文献,检索日期截至2012年10月.由两名研究者分别独立检索文献、评价文献质量、提取资料并交叉核对.利用Stata 10.1统计学软件采用固定或随机效应模型计算优势比(odds ratio,OR)和95%置信区间(95% confidence interval,95% CI)对TNF-α -238G/A、-308G/A和-863C/A多态性与冠心病易感性的关系进行总体评估.结果 共有8篇文献(包含14项独立研究)纳入当前的Meta分析,所有文献研究对象均源自汉族人群.Meta分析结果 显示,TNF-α -238G/A和TNF-α -308G/A多态性与中国人群冠心病的发生无显著相关性(P ＞ 0.05),然而携带TNF-α -863A等位基因(基因型CA或AA)的个体发生冠心病的概率降低了32%(OR = 0.68,95% CI = 0.51~0.90,P = 0.007).结论 中国汉族人群中TNF-α -863C/A多态与冠心病易感性显著相关,但仍需在大样本量的病例对照研究中进一步验证.%Objective To assess the association of polymorphisms in promoter region of tumor necrosis factor (TNF)-α (including-238G/A, -308G/A and -863C/A) with risk of coronary heart disease (CHD) in Chinese population by Meta analysis. Methods Related articles were identified by searching in PubMed, Wanfang and VIP databases before October 2012. Two investigators independently searched databases, extracted data and crosschecked. Combined OR and 95% CI were calculated by fixed-model or random model to assess the association of the three polymorphisms with CHD susceptibility. All statistical analyses were performed using Stata 10.1 statistical software. Results 8 articles (including 14 independent studies) were enrolled in the present Meta-analysis, and all the subjects were Chinese Han ethnic. The result of Meta-analysis indicated that TNF-α -238G/A and TNF-α -308G/A polymorphisms were not associated with incidence of CHD in Chinese population (P > 0.05). However, the probability of individuals carrying TNF-α -863A allele (genotype CA or AA) suffered CHD reduced 32% (OR=0.68, 95% CI=0.51-0.90, P = 0.007). Conclusion The TNF-α-863C/A polymorphism is significantly related with CHD susceptibility in Chinese population, and the findings should be verified with case-control studies involving large number of samples.
