地佐辛联合依托咪酯用于肝癌射频消融术的效果

摘要

目的 探讨地佐辛联合依托咪酯用于肝癌射频消融术患者的麻醉效果.方法 选取2015年9月～2016年9月辽宁省朝阳市中心医院采用射频消融手术治疗的120例肝癌患者,根据随机数字表法将患者随机分为A组(地佐辛+依托咪酯)、B组(舒芬太尼+依托咪酯),每组各60例.监测两组患者麻醉诱导前(T0)、麻醉诱导后(T1)、热疗中(T2)、退针时(T3)、苏醒时(T4)的心率(HR)、动脉血氧饱和度(SpO2)、平均动脉压(MAP)的变化情况,记录患者的苏醒时间、拔管时间、Ramsay评分及其不良反应.结果 T0时刻,两组患者HR、SpO2、MAP水平比较,差异无统计学意义(P＞0.05).在T2、T3时刻,B组HR水平均高于A组,差异有统计学意义(P＜0.05).在T1、T2、T3时刻,B组MAP均低于A组,差异有统计学意义(P＜0.05).A组苏醒时间、拔管时间均短于B组,差异均有统计学意义(P＜0.05).两组Ramsay评分比较差异无统计学意义(P＞0.05).A组总不良反应发生率(10.00％)明星低于B组(25.00％),差异有统计学意义(P＜0.05).结论 地佐辛联合依托咪酯用于肝癌射频消融手术的麻醉效果可靠,对患者血流动力学影响较小,不良反应发生率低.%Objective To investigate the anesthetic effect of Dezocine combined with Etomidate in patients undergoing radiofrequency ablation (RFA).Methods One hundred and twenty cases with liver cancer who were treated with RFA in Chaoyang Central Hospital from September 2015 to September 2016 were selected and randomly divided into group A (Dezocine+Etomidate),group B (Sufentanil+Etomidate) according to random number table method,with 60 patients in each group.The conditions of heart rate (HR),arterial oxygen saturation (SpO2),mean arterial pressure (MAP) of the two groups were monitored before anesthesia induction (T0),after anesthesia induction (T1),heat treatment (T2),needle retraction (T3),wake time (T4).The wake time,extubation time,Ramsay score and its adverse effects were recorded.Results There were no significant differences of HR,SpO2 and MAP between the two groups at To (P ＞ 0.05).At T2 and T3,the HR level of group B was higher than that of group A,the difference was statistically significant (P ＜ 0.05).The levels of MAP in group B were lower than those in group A at T1,T2 and T3,the differences were statistically significant (P ＜ 0.05).The wake time,extubation time in group A was shorter than that in group B,the differences were statistically significant (P ＜ 0.05).There was no significant difference in Ramsay score between the two groups (P ＞ 0.05).The total incidence of adverse reactions in group A (10.00％) was lower than that in group B (25.00％),the difference was statistically significant (P ＜ 0.05).Conclusion Dezocine combined with Etomidate for anesthesia of liver cancer RFA surgery has reliable anesthetic effect,which has less effects on the haemodynamicst,with low incidence of adverse reactions.
