高血压患者幸福感指数及抑郁状况的相关研究

         

摘要

Objective To investigate the well-being index and depression status of patients with hypertension,understand the influence of depression on patients with hypertension by retrospectively analysis.Methods 114 patients with hypertension from September 2015 to March 2016 in Cardiac Department of Renmin Hospital of Wuhan University were analyzed retrospectively.Patients' social and demographic information,including age,gender,marital status,etc were reocorded.Hamilton depression scale (HAMD) and Happiness index scale were taken to access depression status and well-being index of the patients.SPSS 20.0 was used to analyze depression and well-being index status of patients with hypertension.Results The HAMD scores had significant difference in different place of residence (P < 0.05),education level (P < 0.05),monthly income (P < 0.05).The well-being index scale score had significant difference in different cultural levels (P < 0.05).The 114 cases of hypertensive patiets were divided into four groups,namely normal group,possibely depression group,depression group and major depression group,which had 42,43,28 and i cases respectively,and accounting for 36.84%,37.72%,24.56% and 0.9%.The overall sentiment score had statistically difference among groups (P < 0.05).There were no significant differences in overall life satisfaction (P > 0.05) and well-being index scores among groups (P > 0.05).Diurnal variation (r=0.225),hopeless (r=0.240) were positively correlated with overall satisfaction significantly (P < 0.05);didrnal variation (r=-0.210) was negatively significantly correlated with the overall sentiment index (P < 0.05);cognitive impairment (r=-0.281),slow (r=-0.342),hopelessness (r=-0.352) were negatively significantly correlated with the overall sentiment index (P < 0.01).Conclusion Depression status in patients with hypertension significantly correlate with its overall sentiment index.As a medical staff,should pay close attention to the psychological condition of patients,imply the patient-centered holistic nursing and follow bi-psychosocial model,in order to improve depression status and enhance the well-being of patients,improve patients' satisfaction and promote the rehabilitation of patients with hypertension.%目的 探讨高血压患者幸福感指数及抑郁状况,通过回顾性分析了解抑郁对高血压患者幸福感的影响.方法 回顾性分析2015年9月~2016年3月在武汉大学人民医院心内科住院的高血压患者114例,记录患者的社会人口学资料,包括年龄、性别、婚姻状况等.采取汉密尔顿抑郁量表(HAMD)和幸福感指数量表评定患者的抑郁和幸福感情况.运用SPSS 20.0对高血压患者幸福感指数及抑郁状况予以分析.结果 汉密尔顿抑郁量表总分在不同居住地、文化程度、月收入、性别间差异有统计学意义(P<0.05);幸福感指数量表总分在不同文化程度间差异有统计学意义(P<0.05).将114例高血压患者分为4组,即正常组、可能有抑郁组、肯定抑郁组、严重抑郁组,其中正常42人,可能有抑郁43人,肯定抑郁28人,严重抑郁1人,分别占36.84%、37.72%、24.56%、0.9%.各组间总体情感指数差异有统计学意义(P<0.05),各组间生活总体满意度差异无统计学意义(P>0.05),各组间幸福感指数差异无统计学意义(P>0.05).日夜变化(r=0.225)、绝望感(r=0.240)与总体满意度呈显著正相关(P< 0.05);日夜变化(r=--0.210)与总体情感指数呈显著负相关(P<0.05);认知障碍(r=-0.281)、迟缓(r=-0.342)、绝望感(r=-0.352)与总体情感指数呈显著负相关(P<0.01).结论 高血压患者抑郁程度与其总体情感指数和幸福感指数显著相关,作为医务工作者,应密切关注患者心理状况,实施以患者为中心的整体护理,以生物-心理-社会模式服务患者,改善高血压患者抑郁状况,提升患者幸福感,提高患者满意度,促进高血压患者的康复.
