18F-FDG PET-CT全身显像与CT平扫+增强对喉癌患者病变程度及临床分期的诊断价值比较

摘要

目的:分析18F-氟代脱氧葡萄糖 (18F-FDG) PET-CT全身显像与CT平扫+增强对喉癌患者病变程度及临床分期的诊断价值.方法:选取医院收治的92例喉癌患者, 入院后均接受18F-FDG PET-CT全身显像与CT平扫+增强检查, 对比两种不同检查方式下喉癌患者诊断灵敏度、特异度及准确率的差异.结果:18F-FDG PET-CT对原发病灶、淋巴结转移诊断灵敏度、特异度及准确率均显著高于CT平扫+增强诊断结果, 其差异有统计学意义 (x2=14.687, x2=11.442, x2=14.541;P<0.05) ;在92例喉癌患者中经PET-CT显像后29例患者临床分期均发生改变, 27例患者临床分期提高, 2例患者分期降低;T1期、T2期、T3期和T4期淋巴结转移率差异均有统计学意义 (x2=13.843, x2=5.170, x2=4.221, x2=39.000;P<0.05) .结论:相较于CT平扫+增强检查, 18F-FDG PET-CT全身显像对喉癌患者病变程度及临床分期具有较高的诊断价值, 适合临床诊断应用.%Objective:To analyze the diagnostic value of 18F-FDG PET-CT whole body imaging and CT plain scan with enhancement on the lesion level and clinical staging of patients with larynx cancer.Methods:92 patients with larynx cancer were enrolled in this research and all of them underwent 18F-FDG PET-CT whole body imaging and CT plain scan with enhancement after they admitted in hospital.The differences of diagnostic sensitivity, specificity and accuracy of patients with laryngeal cancer between two different examinations were compared.Results:The diagnostic sensitivity, specificity and accuracy of 18F-FDG PET-CT in primary lesions and lymphatic metastases were significantly higher than those of CT plain scan with enhancement (x2=14.687, x2=11.442, x2=14.541, P<0.05) .After 92 patients with laryngeal cancer underwent PET-CT imaging, the clinical stages of 29 patients were changed, and the clinical stages of 27 patients were■on lesion level and clinical staging of patients with laryngeal cancer, and it is more suitable in the application of clinical diagnosis.
