高频彩色多普勒超声对早期乳腺导管癌诊断价值研究

     

摘要

目的:探讨与分析高频彩色多普勒超声检查对早期乳腺导管癌的诊断价值.方法:选取在医院接受治疗的370例乳腺导管癌患者,将其中的270例早期乳腺癌浸润性导管癌患者分为观察组,将100例乳腺原位导管癌患者分为对照组.经高频彩色多普勒超声检查,观察两组患者肿块部分的形态、特征、纵横比例、钙化灶、血流现象及内部回声等指数.结果:观察组患者血流信号强度显著高于对照组患者,两组0~Ⅲ级的血流信号比较,差异有统计学意义(Z=8.422,Z=12.241,Z=11.543,Z=12.256;P<0.05),且观察组患者血流速度显著快于对照组患者,其阻力指数显著低于对照组,差异有统计学意义(t=19.741,t=12.351;P<0.05),乳腺癌浸润性导管癌患者超声图像多表现为边缘形态不规则,后方回声无衰退,侧壁有明显声影,微小钙化灶清晰,且多数图像纵横比>1.两组患者的诊断灵敏度和特异度及准确率诊断差异无统计学意义(x2=0.593,x2=0.868,x2=0.544;P>0.05).结论:应用高频彩色多普勒超声检测,结合患者血流动力学特征与图像表现进行判定与诊断,对早期乳腺癌浸润性导管癌的诊断有十分重要的意义.%Objective:To explore and analyze the diagnosis value of high frequency color Doppler ultrasound for the early breast ductal carcinoma.Methods: 370 patients with breast ductal carcinoma were enrolled in this study, and 270 cases with early invasive ductal carcinoma were divided observation group, while 100 cases with ductal carcinoma in situ were divided into control group. The shapes, characteristic, the ratio of vertical and horizon, calcification, bleeding phenomenon and internal echo of the lumps in two groups were observed through the detection of high frequency color Doppler ultrasound.Results: The blood flow signal intensity of breast cancer patients with invasive ductal carcinoma was significantly higher than that of the control group(Z=8.422,Z=12.241,Z=11.543,Z=12.256,P<0.05), and the blood flow velocity of the patients with invasive ductal carcinoma was significantly faster than that of the control group, while the resistance index of observation group was significantly lower than that of the control group (t=19.741,t=12.351,P<0.05). The most of ultrasound images of breast cancer patients with invasive ductal carcinoma showed irregular margin shape, no fading of the rear echo, obvious sound shadow on the side wall and the clear picture of micro calcification. Moreover, the ratios of vertical and horizon in most of the images were more than 1. The differences of sensitivity, specificity and accuracy rate between the two groups were no statistically significant (x2=0.593,x2=0.868, x2=0.544, P>0.05).Conclusion: The detection of high frequency color Doppler ultrasound combine with the characteristic of haemodynamics and the appearance of image are applied to determine and diagnose early invasive ductal carcinoma have very important values.
