胸膜病变超声引导下穿刺活检价值及并发症研究

摘要

目的 分析超声引导下经皮穿刺活检胸膜占位性病变的常见并发症,探讨其临床价值.方法 34例胸膜占位患者,在常规超声引导下经皮穿刺活检,所取组织行常规病理学检查,部分做免疫组化分析及抗酸染色.结果 本组患者穿刺成功率100％,定性诊断率94.1％,符合率84.4％.并发症发生率8.8％,主要并发症包括:少量气胸1例(2.9％)、咯血1例(2.9％)、胸膜反应1例(2.9％),予以处理后均好转.炎症、结核及肿瘤转移具有各自超声表现.结论 超声引导下经皮胸膜穿刺活检是一项安全有效的诊断方法,可以安全简便地提高胸膜占位病变的诊断准确率.%Objective To study the significance and complications of ultrasound-guided percutaneous biopsy in pleura space-occupying lesions.Methods Percutaneous biopsies are performed under ultrasound guidance in 34 patients with pleura lesions.All specimens are performed with routine pathological examination and parts of them are analyzed in immunohistochemical and acid-fast staining methods.Results The success rate is 100％; diagnosis rate is 94.1％; the coincidence rate is 84.4％; the complication rate is 8.8％.Complications include 1 case of pneumothorax(2.9％),1 case of hemoptysis (2.9％) and 1 case of pleural reaction (2.9％).All complications are relieved after treatment.Characteristics of inflammation,tuberculosis and neoplasm metastasis are displayed respectively on ultrasonography.Conclusion Ultrasound-guided percutaneous biopsy,as a safe and effective method,can enhance the accuracy in diagnosis of pleura lesions.
