目的：探讨360°环阵探头经直肠腔内超声在直肠癌术前TN分期中的应用价值。方法采用360°环阵探头时60例直肠癌患者进行术前经直肠腔内超声检查，然后进行术前超声TN分期（UTN），并与手术病理分期（PTN）结果进行对照。结果环阵探头经直肠腔内超声检查对直肠癌术前T分期的总准确率为75％（45／60），PT1、PT2、PT3、PT4各期的准确率分别为84.6%（11/13）、70.6％（12／17）、70.0％（14／20）、80％（8／10），对淋巴结转移的诊断敏感性、特异性及准确性分别为74%（20／27）、76%（25／33）及75%（45／60）。结论采用环阵探头经直肠腔内超声检查可以比较准确地对直肠癌进行术前TN分期，是一种有效的术前诊断方法，对直肠癌患者手术方式的选择有指导意义。%Objective To investigate the value of TRUS (Transrectal Ultrasonography) with annular 360 array probes in the preoperative TN staging of rectal carcinoma. Methods Sixty cases of clinically-diagnosed rectal carcinoma were staged pre-operatively by TRUS with annular 360 array probes and compared with postoperative pathological staging respectively. Results The overall accuracy ratio of TRUS with annular array probes was 75%(45/60). The accuracy of TRUS for PT1, PT2, PT3 and PT4 tumors was 84.6%(11/13), 70.1%(12/17), 70.0%(14/20) and 80%(8/10) respectively. The sensitivity, speciifcity and accuracy of TRUS in lymph node metastasis were 74%(20/27), 76%(25/33) and 75%(45/60) respectively. Conclusion As an effective pre-operative diagnosis method, TRUS with annular array probes demonstrated its accuracy in preoperative staging of the rectal carcinoma, which offered important evidences for clinical diagnosis and treatment of the rectal carcinoma.
