实时弹性成像技术对不同功能状态桥本甲状腺炎的评估价值

         

摘要

Objective To investigate the value of real-time elasticity imaging in the diagnosis of Hashimoto thyroiditis (HT) with different functional thyriod. Methods 120 patients clinically diagnosed with HT in our hospital from September 2014 to April 2015 were selected. Based on the results of the thyroid function tests, these patients were divided into three groups: the hyperthyroidism group (28 cases), the euthyroidism group (40 cases), and the hypothyroidism group (52 cases). 50 healthy volunteers were enrolled to the control group. All patients were examined by traditional ultrasound examination and then real-time tissue elastography. The elastic parameters of LF and AREA% in region of interest were calculated. Correlation analysis between the two indexes and TSH were also performed. ResultsLF and AREA% in HT groups were larger than those in the control group (F=51.54,P=0.000). The two parameters in HT groups were both significantly different (F1=47.54,P1=0.000;F2=38.26, P2=0.011) and were correlated with TSH (r1=0.815,r2=0.64,P<0.01).Conclusion Real-time elasticity imaging may provide a new way to distinguish different functional HT.%目的:探讨实时弹性成像技术对不同功能状态桥本甲状腺炎(HT)的评估价值。方法选择2014年9月~2015年4月于我院经临床确诊的120例HT患者,根据甲状腺功能检查结果分为HT甲亢组(28例)、HT甲功正常组(40例)、HT甲减组(52例),另选50例健康志愿者作为对照组。所有受检者均先经常规超声检查,然后采用实时弹性成像技术测量感兴趣区(ROI)的弹性参数:肝纤维化指数(LF)、蓝色区域面积百分比(AREA%),并分析其与血清促甲状腺素(TSH)的相关性。结果 HT组的LF、AREA%大于对照组,差异具有统计学意义(F=51.54, P=0.000);HT各组间比较,LF、AREA%具有统计学差异(F1=47.54,P1=0.000;F2=38.26, P2=0.011),且均与TSH具有相关性(r1=0.815,r2=0.64,P<0.01)。结论实时弹性成像技术为超声评价不同功能状态HT提供了新的可能。
