卵巢移位术对宫颈癌患者放疗后卵巢功能影响

         

摘要

[Objective] To investigate the changes of ovary' function on iaparoscopic ovarian transposition in young patients with late squamous cervical carcinoma before radio therapy. [Methods] Thirty-four patients are or younger than forty years old with late squamous cervical carcinoma were treated with Iaparoscopic ovarian transposition before radio therapy were regarded as study group. Twenty-eight patients are or younger than forty years old without Iaparoscopic ovarian transposition before radio therapy were regarded as control group. The levels of blood E2, FSH, LH were assessed respectively before operations (and radiations) and 12 months after radiations, and the menopause symptoms of the 62 women were observed 12 months after radiations. The levels of E2, FSH, LH and the rate of the menopause symptoms of two groups were compared and statistics analyzed. [Results] The levels of blood FSH, LH and E2 showed no significant difference before operations in study and before radiations in two groups (P > 0.05), and showed significant difference in 12 months after radiations in two groups (P <0.05). Nineteen cases had declined ovarian function in study group (19/34, 55.9%), and two cases had declined ovarian function, and twenty rnsix were ovarian dysfunction in control group. [ Conclusions ] Patients with late cervical carcinoma with laparoscopic ovarian transposition before radio therapy can reserve ovarian function.%目的 探讨晚期宫颈癌患者在放射治疗前行腹腔镜下卵巢移位术对卵巢内分泌功能的影响.方法 选择40岁以下晚期宫颈鳞癌患者34例为研究组,在放射治疗前行腹腔镜下卵巢移位术.分别于术前,放疗前,放疗后12个月测患者卵泡期血清E2、FSH和LH水平,观察其围绝经期症状出现情况;同期确诊的40岁以下晚期宫颈鳞癌患者,未接受卵巢移位术直接放疗者28例为对照组,分别于放疗前、放疗后12个月测患者卵泡期血清E2、FSH和LH水平,观察其围绝经期症状出现情况.分析两组放疗前后激素水平变化及围绝经期症状发生率.结果 研究组术前,放疗前和对照组放疗前患者的E2、FSH和LH水平均在正常范围内,差异无显著性(P>0.05);研究组放疗后12个月内19例出现卵巢功能下降(19/34,55.9%),对照组则2例出现卵巢功能下降,26例出现卵巢功能丧失,两组间激素水平差异有显著性(P<0.05).结论 年轻晚期宫颈癌患者放射治疗前行腹腔镜下卵巢移位术,可使卵巢功能得到一定的保护.
