儿童睑缘炎相关性角膜病变的临床特点分析

摘要

目的 对儿童睑缘炎相关性角膜病变(BKC)的临床特点进行观察总结,以提高对儿童BKC的关注.方法 回顾性病例研究.收集诊断为BKC的儿童48例(88只眼),对其临床症状,睑板腺分泌物性状以及角膜病变范围进行评分,分析不同性别间的差异.采用相关性分析对角膜病变范围评分与主诉症状和睑板腺分泌物形状评分进行分析.结果 儿童BKC的临床症状评分:1分,3例(6.3％);2分,12例(25％);3分,17例(35.4％);4分,10例(20.8％);5分,6例(12.5％).睑板腺分泌物性状评分分别为,1分,50只眼(56.8％);2分,33只眼(37.5％);3分,5只眼(5.7％).角膜病变范围评分分别为,1分,45只眼(51.1％);2分,30只眼(34.1％);3分,12只眼(13.6％),4分,1只眼(1.1％).不同性别间临床症状评分,睑板腺分泌物性状评分以及角膜病变范围评分差异均无统计学意义(P＞0.05);角膜病变范围评分和临床症状评分,睑板腺分泌物性状评分呈正相关(r=0.35,r=0.80,P＜0.05).结论 BKC发病在本研究中无性别差异,临床症状表现多样,而角膜病变可能与长期的睑板腺分泌异常有关.%OBJECTIVE To pay attention to Blepharokeratoconjunctivitis (BKC) in children by summarizing Clinical characteristics.METHODS Retrospective case study.The clinical data of 48 cases (88 eyes) diagnosed with BKC were collected.The clinical symptoms,meibomian gland secretion traits,scope of Corneal lesions were scored and compared between two genders by Chi-square test.The correlation of the clinical symptoms,secretion traits of meibomian gland and scope of corneal lesions were analyzed by Person correlation analysis.RESULTS The Clinical symptoms scores of BKC in children were 3 cases (6.3％) with 1 point,12 cases (25％) with 2 points;17 cases (35.4％) with 3 points;10 cases (20.8％) with 4 points and 6 cases (12.5％) with 5 points.The scores of meibomian gland secretion traits were 50 eyes (56.8％) with 1 point;33 eyes (37.5％) with 2 points and 5 eyes (5.7％) with 3 points.The scores of corneal lesions were 45 eyes (51.1％) with 1 point;30 eyes (34.1％) with 2 points;12 eyes (13.6％) with 3 points;and 1 eye (1.1％) with 4 points.There were no significant difference between two genders (P＞0.05).The corneal lesion scores were positively related to clinical symptoms scores and meibomian gland secretion traits (r=0.35,0.80,P＜0.05).CONCLUSIONS BKC had similar characteristics in both boys and girls,but showed various clinical symptoms.The corneal lesion were possibly related with meibomian gland abnormalsecretion.
