衍射型非球面双焦点人工晶状体植入术后综合视觉质量分析

     

摘要

OBJECTIVE This paper aims to study the visual quality of patients with implantation of the diffractive aspherical bifocal intraocular lens(BIOL). METHODS Retro-spective non-randomized study was implemented. In clinical application from October 2016 to February 2017, 23 patients (34 eyes) were implanted with the diffractive aspherical bifocal IOL (ZEISS AT LISE 809M) via the phacoemulsification. Three months after the surgeries, data were recorded regarding the uncorrected distance visual acuity (UCDVA, 5m), the uncorrected near vi-sual acuity (UCNVA, 33cm), the best corrected distance visual acuity (BCDVA), the best corrected near visual acuity (BCNVA), the Optical Quality Analysis System (OQAS), as well as the Modula-tion Transfer Function (MTF cutoff), the Objective Scattering Index (OSI), the Strehl Ratio (SR), defocus testing, questionnaire scores, and comprehensive evaluation of postoperative visual quality of observed eyes. RESULTS Compared to the pre-operation without the implantation, both the av-erage UCDVA (0.04 ±0.08) with (t=-12.499, P<0.001) and the average UCNVA (0.21 ±0.12) with (t=-17.772, P<0.001) showed better performance. Besides, the average MTF cutoff (28.35 ±10.40) c/deg presented obvious improvement compared with the pre-surgery index (14.15 ±9.56) c/deg. The difference indicated statistically significant (t=-7.094, P<0.001). The average OSI of pre-op-eration (3.69±2.51) was higher than the post-operation value (2.70±1.64). Again, the difference in-ferred statistically significant (t=2.430, P=0.020). Pre-operation average SR (0.10±0.05) was lower than the post-operation (0.16±0.07), which expressed statistically significant (t=-4.918, P<0.001). Index of the spectacle independence reached 88.24%after the operation. CONCLUSIONS The diffractive aspherical bifocal IOL ( ZEISS AT LISE 809M) implantation can improve visual quality of cataract patients, resulting in higher spectacle independence. However, visual disturbance still exists which might indicate further improvement.%目的 观察衍射型非球面双焦点(ZEISS AT LISE 809M)人工晶状体植入术后患者综合视觉质量.方法 回顾性非随机研究.研究对象为2016年10月-2017年2月接受白内障超声乳化联合衍射型非球面双焦点人工晶状体(ZEISS 809)植入术的患者23例(34只眼),术后随访3个月.手术前后检查裸眼及矫正远、近视力,客观视觉质量分析系统(O-QAS)测量调制传递函数截止频率(MTF cut off)、客观散射指数(OSI)、Strehl比值(SR),术后测定离焦曲线,并填写满意度及视觉干扰调查表,综合评价被观察眼的术后视觉质量.结果 术后3个月最佳裸眼远视力(log MAR)0.04±0.08,优于术前(t=-12.499,P<0.001),近视力(log MAR)0.21±0.12,优于术前(t=-17.772,P<0.001).术前被观察眼的MTF cut off为(14.15±9.56)c/deg,术后为(28.36±10.40)c/deg,优于术前(t=-7.094,P<0.001).术前OSI 3.69±2.51,术后数值为2.70±1.64,优于术前(t=2.430,P=0.020).术前SR 0.10±0.05,术后0.16±0.07,优于术前(t=-4.918,P<0.001).脱镜率88.24%.结论 衍射型双焦点(ZEISS AT LISE 809M)人工晶状体植入术后视觉质量明显改善,且有较高脱镜率,但仍有视觉干扰.
