临床药师参与急性心肌梗死临床路径工作模式探讨

Objective To explore the clinical pharmacists to participate in the work pattern of clinical pathway.Methods linical pathway method with reference to the ministry of health issued by the form, the content of the pharmaceutical care for the lack of perfect.Results To improve the clinical path form, build the medical treatment team, according to the clinical pharmacist participating in clinical work can depend on is indispensable.Conclusion Containing the clinical pharmacists work content, the process of clinical pathway form deep clinical pharmacists work, not only can open more can make the standardization of clinical pharmacist participating in clinical work, promote clinical pharmacists service ability.%目的 探讨临床药师参与临床路径的工作模式.方法 参照卫生部下发的临床路径表单,对其中缺失的药学服务内容进行完善.结果 完善了临床路径表单,构建了医药护治疗团队,使临床药师参与临床工作有据可依并不可或缺.结论 含有临床药师工作内容、流程的临床路径表单不仅可打开深层次的临床药师工作切入点,更可使临床药师参与临床工作规范化,提升临床药师服务能力.
