目的：探讨影响夫精宫腔内人工授精（IUI）妊娠结局的相关因素，提高受孕率。方法本中心2015年1～12月收治了502例不孕女性，均行夫精IUI，将66例受孕者设为受孕组，436例未孕者为未孕组，对年龄、不孕原因、不孕年限、促排卵方案、IUI治疗周期等进行单因素分析，并以受孕为自变量，各种影响因素为因变量，进一步行多元Logistic回归分析。结果单因素分析显示，年龄≥36岁、不孕年限≥4年、男性少弱精、排卵障碍、自然周期、精子活动率＜80%、治疗周期≥3个周期是影响IUI妊娠结局的因素（P＜0.05）。多元Logistic回归分析显示，年龄≥36岁、男性少弱精是影响IUI妊娠结局的主要因素，而促排卵周期是提高IUI妊娠结局的保护因素。结论行IUI应尽量与促排卵方案联合治疗，可提高受孕率。年龄大、夫少弱精患者，在治疗3个IUI周期失败后，建议考虑其它辅助生殖技术，以提高受孕率。%Objective To explore the related factors that affect the pregnancy outcome of IUI,and increase the pregnancy rate.MethodsFrom January 2015 to December 2015,502 cases of infertile women were selected. All of them were underwent IUI,66 cases of the pregnancy women as the pregnant group,436 cases of non pregnant women as the non pregnant group,single factor analysis of age,cause of infertility,infertility, ovulation induction,IUI treatment cycle,and to conceive for self change the amount,the influence of various factors as dependent variables, further multivariate Logistic regression analysis. ResultsThe Univariate analysis showed that age over 36 years,more than 4 years of infertility, asthenospermia,ovulation disorder,natural cycle,sperm activity rate<80%,treatment cycle more than 3 cycles were factors influencing the pregnancy outcome in IUI(P<0.05). Multivariate Logistic regression analysis showed that age more than 36 years old,asthenospermia is the main factor affecting the pregnancy outcome in IUI,and the cycle of ovulation was a protective factor for improving pregnancy outcome in IUI. Conclusion The combined treatment of IUI should be combined with ovulation induction regimen,which can increase the pregnancy rate. Older age,less than patients with poor sperm,after the treatment of 3 IUI cycles,the proposed consideration of other assisted reproductive technology,to improve the pregnancy rate.
