Key Points for Clinical Pharmacists to Develop Pharmaceutical Care for Anticoagulation in patients Receiving Warfarin to explore the application of anticoagulant therapy clinical pharmacists to participate in anticoagulation management breakthrough point, in the process of standard pharmaceutical care mode.Methods combined with anticoagulation, authoritative guide to anticoagulation drugs in the treatment of patients with pharmaceutical care, ways and methods are analyzed and discussed. Results the anticoagulation in patients with pharmaceutical care, to establish the standardization of the clinical pharmacist participating in clinical anticoagulant management mode.Conclusion clinical pharmacists should from the individual perspective, reasonable use of anticoagulant drugs, to better ensure patients’medication safety.%目的：探讨应用抗凝治疗过程中临床药师参与抗凝管理的切入点，规范药学监护模式。方法结合抗凝权威指南，对抗凝药物治疗患者的药学监护点，途径和方法进行分析和讨论。结果通过对抗凝患者的药学监护，建立临床药师参与临床抗凝管理的规范化模式。结论临床药师应从个体化角度出发，合理应用抗凝药物，更好地保证患者的用药安全。
