目的探讨基层医院实施全程责任制助产加导乐陪伴分娩对提高产科质量的影响。方法回顾性分析2014年1～12月在我院产检和分娩的初产妇，选择上半年、下半年各200例作为分析对象，上半年为对照组，下半年为实验组，对照组采取单纯的全程责任助产手段，实验组在对照组基础上加以导乐陪伴助产，对比两组分娩情况与分娩结局。结果实验组产妇的剖宫产率、会阴侧切率、总产程时长、新生 儿窒息发生率、产后出血量明显降低，产妇满意度明显提高，两组比较差异显著（P＜0.05）。结论基层医院实施全程责任制加导乐陪伴的分娩可提升产科质量，促进自然分娩。%Objective To explore the implementation of midwifery full responsibility system and the inlfuence of leads accompanies on improving the obstetric quality in grass-roots hospitals. Methods Retrospective analysis of our hospital for check and childbirth primipara in 2014 1~12 months, selected 200 cases as the object in the first half of the year and the second half for analysis, the patients in ifrst half of the year were taken as the reference group, the patients in the second half of the year were taken as the experimental group, the reference group received take full responsibility midwifery means, the experimental group received leads accompanies midwifery on the basis of the reference group, compared two groups of delivery and delivery outcome. Results The rate of cesarean section, lateral episiotomy maternal experimental group cut rate, total labor time length, neonatal asphyxia, postpartum hemorrhage volume were decreased obviously, maternal satisfaction improved significantly, the difference between the two groups was significant (P<0.05).Conclusion The implementation of full responsibility system combined with leads accompanies delivery can improve the obstetric quality in primary hospitals.
