椎动脉-小脑后下动脉动脉瘤的血管内治疗

摘要

目的 总结椎动脉-小脑后下动脉动脉瘤的栓塞治疗方法和临床效果.方法 回顾分析2009年1月至2009年7月4例椎动脉-小脑后下动脉动脉瘤患者采取血管内栓塞治疗病例.结果 4例行血管内栓塞病例,达完全栓塞3例,90%栓塞1例.其中1例儿童患者术后3 d出现神经功能障碍,予气管切开,呼吸机辅助呼吸等治疗1个月后康复.术后长期随访至今,GOS预后评分均为良好.结论 血管内治疗是治疗椎动脉-小脑后下动脉动脉瘤的有效方法,采用合适的操作策略可以提高治疗的效果.%Objective To conclude technique and outcome of endovascular treatment for aneurysms of the vertebral artery - posterior inferior cerebellar artery. Methods The endovascular treatment of 4 patients with aneurysms of the vertebral artery - posterior inferior cerebellar artery were analyzed retrospectively from January to July in 2009. Results In the series, total occlusion was obtained in 3 aneurysms and more than 90％ occlusion in 1 case. One case has been symptoms of the bran stem and cranial nerve envolvement. All patients were evaluated as excellent and good by GOS in 2 - 7 months after operation. Conclusion As one of the main treatments for aneurysms of the vertebral artery - posterior inferior cerebellar artery, the endovascular method has its own advantages. Exact endovascular therapeutic strategy is the key for good and excellent outcome.
