特发性全身性强直-阵挛发作患儿及同胞的智商研究

摘要

Objective To study the intelligence quotient (IQ) profile of the children with idiopathic generalized tonic-clonic seizure(GTCS)and their siblings. Methods Eighty-six children in mountain area of the south of Anhui Province were selected and divided into three groups ,28 children with GTCS who were aged 9 ～ 16 years in 20 epilepsy families constituted GTCS group, all 28 available healthy siblings of the children with GTCS, did not have epilepsy, and were nearest in age to the child with GTCS constituted sibling group, and thirty children of age and sex matched who had lived in the same village constituted control group. The IQ of the three groups of children was studied. Results Compared with sibling group and control group, the IQ of 47％ of the children with GTCS was lower than average range( IQ <90),the difference was significant (P <0. 01 ). The IQ of 32％of the children with GTCS was within mean range( IQ 90 ～ 109 ). The IQ of 21％ of the children with GTCS was higher than mean range (IQ≥109) ,was lower than that of sibling group and control group. The mean Full Scale lQ of children with GTCS was lower than that of sibling group and control group. The difference among children with GTCS and the sibling group and control group were significant for almost all the subtest quotients except completion of drawing and picture arrangement. There was no significant difference between the sibling group and the control group on the IQ and the subtest quotients. Conclusion The IQ scores of the children with GTCS are lower significantly than that of their siblings and unimpaired children, but there is no significant difference between the siblings of the children with GTCS and the unimpaired children on the IQ level.%目的 了解特发性全身性强直-阵挛发作(GTCS)患儿及同胞的智商情况.方法 选择皖南山区86例儿童作为研究对象,分3组:GTCS组为20个癫痫家系中28例9～16岁GTCS患儿,同胞组为28例与GTCS患儿年龄差距最小的健康同胞,对照组为30例年龄、性别相匹配的同村健康儿童.研究3组儿童的IQ水平.结果 47% GTCS患儿IQ低于平均水平(IQ＜90),与同胞组和对照组相比,差异有统计学意义(P＜0.01);32% GTCS患儿IQ位于平均水平(IQ 90～109),21%高于平均水平(IQ≥109),低于同胞组及对照组.GTCS患儿的平均总智商比同胞组及对照组均低.除了填图与图片排列外,GTCS患儿的所有其他分测验得分均低于同胞组及对照组.同胞组及对照组之间的IQ水平及各分测验得分差异无统计学意义.结论 GTCS患儿IQ明显低于其无癫痫发作的同胞及正常儿童,而GTCS患儿无癫痫发作的同胞与正常儿童的IQ无明显差异.
