检测血浆中p16及CDO1基因甲基化在胶质瘤诊断中的价值

     

摘要

采用甲基化特异PCR(MSP)法分别检测49例胶质瘤患者组织及其对应的术前血浆,10例颅脑外伤患者挫伤的脑组织及其对应的术前血浆和16例健康体检者血浆中p16及半胱氨酸双加氧酶1(CDO1)基因的甲基化状态,以探讨血浆中检测p16及CDO1基因甲基化在胶质瘤临床诊断中的价值.胶质瘤组织中p16及CDO1基因甲基化率分别为53.1%(26/49)、42.9%(21/49),对应的血浆甲基化率为39.0%(19/49)、34.7%(17/49).在肿瘤组织和血浆中p16及CDO1至少一个[p16和(或)CDO1]基因甲基化的频率为75.5%(37/49)和67.3%(33/49).血浆中p16及CDO1基因(甲基化)联合检测胶质瘤的灵敏度为67.3%,较单基因检测的灵敏度明显提高.单因素分析显示术前患者血浆中p16和(或)CDO1基因的甲基化与年龄、性别、肿瘤分级无关.对一些肿瘤特异基因的甲基化状态进行联合检测可提高胶质瘤在血清学诊断水平上的灵敏度,为胶质瘤的筛查及早期诊断提供新的临床诊断思路.%Methylation-specific polymerase chain reaction was used to detect the methylation statuses of gene p16 and gene CDO1 from 49 patients with glioma tissues and their corresponding preoperative plasma, 10 patients with craniocerebral brain tissues their corresponding preoperative plasma, as well as 16 samples of plasma from healthy subjects, respectively, in order to investigate the value of the methylation detection of gene p16 and gene CDO1 from plasma in the clinical diagnosis of glioma.For glioma tissues, the methylation rates of gene p16 and gene CDO1 were 53.1% (26/49) and 42.9% (21/49), respectively, while the methylation rates of the corresponding plasma were 39.0% (19/49) and 34.7% (17/49), respectively.As for tumor tissues and plasma, the frequencies for methylation of gene p16 and/or gene CDO1 were 75.5% (37/49) and 67.3% (33/49).The joint detection of the methylation of gene p16 and gene CDO1 in the plasma for the sensitivity of glioma was 67.3%, an obvious improvement compared with single gene detection.In addition, a univariate analysis showed that the methylation of gene p16 and gene CDO1 from the preoperative plasma had no significant correlation with the patient'age, sex or tumor grade.Therefore, we draw a conclusion that a joint detection of the methylation status of some specific tumor genes can improve the sensitivity of glioma at the serological diagnosis level, which may provide new clinical thoughts for screening and early diagnosis of glioma.
