Objective To examine the efficacy of mini-invasive treatment of chronic dacryocystitis based on the findings of three-dimensional imaging of multi-slice spiral CT. Methods The length of the bony lacrimal duct and the diameter of the narrowest part of the duct were measured by three-dimensional imaging technique.Three groups were established in terms of the following conditions:①the length of the bony lacrimal duct was less than 13.9 mm and ②the narrowest part of the duct was more than 3. 1 mm in diameter. Patients in group A (23 cases) fulfilled both conditions. Those in group B (26 cases) satisfied either of the conditions. None of the conditions were fulfilled in group C (24 cases). The retrograde lacrimal intubation was performed in all the patients of the three groups. Patients who underwent dacryorhinocystostomy served as positive control group (43 cases). Results The healing rate of patients who underwent retrograde lacrimal intubation was significantly less than that in the positive control group (65. 75% vs. 88. 37% ,P<0. 01 ). However ,the healing rate of the group A (82. 61% ) was similar to that of the positive control group (P>0. 05). Conclusion The healing rate of retrograde lacrimal intubation can be significantly improved if chronic dacryocystitis patients whose bony lacrimal duct is short in length and large in diameter are selected by the three-dimensional imaging technique.%目的 探讨应用CT三维成像技术指导微创治疗慢性泪囊炎,以提高手术成功率.方法 通过CT三维成像技术测量骨性泪道的长度及最小直径.依据泪道长度小于13.9 mm及最小直径大于3.1 mm将患者分为3组:同时满足以上条件者为A组(23例),满足其中之一者为B组(26例),未达到以上条件者为C组(24例).3组患者均行泪道逆行置管手术.对照组(43例)采用泪囊鼻腔吻合术.结果患者行泪道逆行置管术的治愈率为65.75%,低于对照组(88.37%)(P<0.01).A组患者手术治愈率(82.61%)与对照组相比较差异无统计学意义(P>0.05).结论通过CT三维成像技术选择泪道长度短、直径大的患者行泪道逆行置管术,可以有效提高手术成功率.
