益气活血方联合三维适形放疗治疗中晚期原发性肝癌临床分析

摘要

目的：观察中药配合三维适形化疗治疗中晚期原发性肝癌近期临床疗效及对相关指标的影响。方法：将原发性肝癌患者63例随机分为治疗组32例和对照组31例。对照组采用三维适形放疗，治疗组在对照组基础上放疗期间和放疗后3个月采用中药治疗。治疗后评价临床疗效、KPS评分、毒副反应等相关指标。结果：肿瘤客观疗效评价：治疗组总有效率为81．25％，对照组总有效率为41．93％，两组总有效率比较具有统计学意义（χ2＝8.72， P ＜0．01）。两组AFP 指标治疗前后无统计学意义（ P ＞0.05）。治疗组CD4＋、CD8＋、KPS评分改善情况优于对照组（P＜0．01）；治疗组3个月、6个月生存率明显高于对照组（P＜0．05）；治疗组发生白细胞降低率明显低于对照组，具有统计学意义（χ2＝3．92， P＜0．05）。结论：中药配合三维适形化疗能够明显改善临床症状，提高患者3个月、6个月生存率，毒副作用小，是治疗中晚期肝癌的有效途径。%Objective:To investigate the clinical effect and related index of Yiqi Huoxue decoction combined with 3D-CRT on advanced primary liver cancer.Methods:Use the digital table method and 63 patients diagnosed with primary liver cancer by clinic were randomized into an observation group ( n=32 ) and a control group ( n=31 ) .The control group was treated with 3 D-CRT, while during and after 3 months of the radiotherapy the observation group was treated with Chinese medicine orally according to their symptoms.The clinical efficacy, KPS score and incidence of adverse re-action were analyzed.Results:The objectively therapeutic evaluation of tumor with the total effective rate was 81.25%in the observation group.By contrast, the total effective rate was 41.93% in the control group.Results showed the difference in the effective rate between two groups was significant (P<0.05).There was no statistical significance in the AFP levels.In two groups, CD4 +, CD8 +and KPS score in the observation group were better than those in the con-trol group ( P<0.01) .For the survival rate, there was a significant difference according to 3 months and 6 months sur-vival rate between two groups(P<0.05), and the survival rate of control group was lower.However, for the incidence of hypoleucocytosis, the results of two groups were also significant ( P<0.05) ,and the observation group was lower than control group.Conclusion:The effect of Chinese medicine combined with 3D-CRT on PLC in advanced stage is better. It can improve the patients’ survival rate of 3~6 months,improving quality of life among PLC patients.Moreover,it can decrease the side effect.This can be considered as one effective method for treating PLC.
