Risk Assessment and Job Safety Analysis techniquesare being used to drive down accident rates ingeographically diverse oil and gas industry sectors. Thispaper focuses on sharing practices that can be used tomake a significant contribution to the safety of tasksduring operations.These learnings and the practices derived are importantin the operational processes of drilling and workoveractivities. Applicable by supervisors and safety leadersat the conceptual stage, immediately prior to workcommencing and whilst work is ongoing, the issues areof critical importance in both development of safetyculture at the sites and in the design of trainingprogrammes. They offer the opportunity to benchmarkHS&E Risk Management systems and allow the deliveryof sustained HS&E performance through the sharing ofbest practices.The practice of applying task-based Risk Assessmenthas been a 'painful' one for drilling Operations with acontinual process of learning and innovation resulting inthe process being increasingly effective. Continuedprogress towards the goal of an incident free workingenvironment can be enhanced by some keyobservations, these centre on how rig-based staff andonshore staff may use the same language, but maymean and practice different things. Existing technologiesmay offer significant opportunities for HS&E riskidentification and management but are themselves onlypart of the jigsaw. Finally, this paper demonstrates howexamination of the people and system practices that contribute to 'excellence in risk assessment', can createa powerful tool in driving towards zero incidents.Conclusions highlight the failures inherent in someexisting HS&E Risk Management approaches where riskmanagement is seen as a 'requirement' or one that thatlegitimizes the unsafe acts. Examples are offered of theinnovations of existing systems outlining the associatedbenefits.
