Broad introduction and development of polymeric materials in abdominal hernia surgery led to the emergence and identification of secondary complications due to interaction between prosthetic material and human tissue. Whether identification of these reactions has led to the placement instructions of the prostheses into the wall structures of abdomen, there is no clear rule on fixation materials of these prosthesis; they generally follow the recommendations of the prosthesis materials.Objective of the paper is to present and classify the risk of infection depending on the structure of the bio-material used for mesh fixation in hernia surgery. There are some situations in which the faulty choice of fixing materials can compromise such surgery, followed by chronic septic complications and negative effect on quality of life of these patients and increased risk of recurrence. The theoretical considerations and experimental studies show that materials susceptibility to infection is directly dependent on the structure, filament and porosity of the material. Choice of fixation biomaterial, according to the structure and topography of the prosthesis may influence the risk of chronic infection and it can prevent the long term complications. As conclusion, we consider that the choice of fixing materials considering the structural characteristics of polymeric threads can prevent chronic suppuration secondary to this type of surgery.
