The construction process and its phases are discussed as it relates to the inception and perpetuation of errors. As suggested by four examples of notorious and recurring mishaps, it appears that some scenarios relate to attitudes inherent in engineering pratice and society at large, rather than to the acts of individuals. In a second paper presented at this same conference, a number of other cases will be presented that are more directly attributable to specific participants of the building process.
