The organizational changes can trigger events causing accidents, especially when there is substitution of key personnel or functional departments that could stop the initiating events of accident process. The lack of appropriate decision-makers and experts, with the dynamism and knowledge required, creating gaps in functions inside the company. On the other hand, the analysis of long-term or short-term to decisions depends of absent job staff. The speed is not appropriate for the integration between maintenance and operation in the industry. The organizational change has greater effect on process safety than technological changes. The organizational changes alter the balance of the work and business which, according to most authors of reliability and safety, is the area of the root cause of the major events (accidents). The organizational changes that can lead to greater fragility in process safety at industrial facilities are: working conditions, personnel headcount, functional re-allocation, organizational hierarchy, and, policy changes that-affect safety culture. It is essential that analyze the loss arising from organizational change to define whararefhe main characteristics desired in leadership who will plan, deploy, and monitor these changes. The proposition is to anticipate the discussion of critical situations caused by organizational changes using a checklist that call a committee composed by internal and external members of the siterin. study. This committee will propose preventive and corrective actions to avoid events in safety process of this industry. In this paper a checklist was answered by staff involved with a ease of organizational change.
